Introduction : Les fractures de l'acétabulum représentent 7% des fractures ostéoporotiques. Leur nombre ne cesse de croître en raison de l'allongement de la durée de vie. Le pic épidémiologique se situe entre 75 et 80 ans. L'objectif de cette étude est d'évaluer les résultats fonctionnels des prises en charge de ces fractures au sein d'une population âgée de plus de 65 ans. Matériel et méthodes : Il s'agit d'une étude monocentrique rétrospective sur un recueil prospectif de données menée au CHU de Dijon. L'ensemble des patients de plus de 65 ans ont été inclus entre janvier 2017 et mai 2020. Le critère de jugement principal est composé d'un score PMA, Harris et Parker. Les critères secondaires sont le taux de mortalité et le taux de reprise chirurgicale. Résultats : 94 patients (âge moyen de 78.5 ans) ont été inclus. Le suivi moyen est de 32.7 mois +/- 14.9 Le PMA moyen à 2 ans : 14.5 pour le groupe traitement orthopédique, 16.5 pour le groupe ostéosynthèse, 16.4 pour le groupe ostéosynthèse + PTH (p = 0.01). Le score de Harris moyen à 2 ans est de 72.1 pour le groupe traitement orthopédique, 85.4 pour le groupe ostéosynthèse, 85 pour le groupe ostéosynthèse + PTH (p = 0.012). Le score de Parker était significativement plus élevé dans les groupes opérés et notamment dans le groupe traité par ostéosynthèse. Le taux de reprise pour conversion en arthroplastie totale était de 24.1% au sein de la population non opérée et de 11.1% pour les patients traités par ostéosynthèse simple. Le taux de complications de 24% pour le groupe ostéosynthèse, et 42% pour le groupe ostéosynthèse + PTH. Les taux de mortalité étaient plus élevés dans les groupes traitement orthopédique et ostéosynthèse + PTH. Discussion : Le traitement orthopédique semble être inférieur au traitement chirurgical en termes de résultats fonctionnels avec un taux de réintervention pour arthroplastie non négligeable. Le traitement par réduction et ostéosynthèse, avec des scores fonctionnels plus élevés, un taux de complications et de reprises plus faibles, représente pour nous le traitement de choix dans ce type de traumatisme. L'effet préventif sur la mortalité reste à déterminer. Conclusion : Le traitement chirurgical des fractures de l'acétabulum permet d'obtenir les meilleurs résultats fonctionnels au sein des populations âgées de plus de 65 ans.
