No two markets for voluntary health insurance (VHI) are identical. All differ in some way because they are heavily shaped by the nature and performance of publicly financed health systems and by the contexts in which they have evolved. This report is a summary of a workshop focused on exploring the role of the public in the Clinical Research Enterprise. The Clinical Research Enterprise depends upon practitioners, policy makers, and others for participation in trials, ethical review of research, and continued support of research funding. However, the role of the public has expanded beyond this traditional model as consumers have begun to demand a role in the formulation of the research agenda and in the design, review, and pursuit of research. This report identifies four major challenges to the Clinical Research Enterprise: enhancing public participation in clinical research, which includes making the system safer and faster; developing the necessary information systems that are needed to make the clinical research enterprise a coordinated and seamless whole; fostering an adequately trained workforce; and ensuring adequate funding for clinical research. In addition, the report identifies two translational blocks-from basic science into clinical practice and from the clinical identification of things that work into broader application to improve medical care and the public's health. This workshop summary addresses the contribution of the public to overcoming these obstacles.

If public resources were unlimited there would be no gaps in health coverage and no real need for VHI. However most health systems face fiscal constraints and VHI is often seen as a way to address these pressures. This study draws from the experiences of 34 countries to assess VHI s contribution to health spending and to understand its role in Europe and in relation to publicly financed coverage. It looks at who sells VHI who purchases it and why. It also reviews public policy towards VHI at national and EU levels and the related national policy debates. The analysis shows that while the different markets for VHI vary considerably in size operation and regulation the vast majority are small. Where there are substantial markets these tend to be the oldest ones having a tradition of non-profit insurers and to be the most heavily regulated to ensure VHI policies are accessible and affordable. The study also suggests that VHI is normally a better way of meeting population health needs than out-of-pocket payments although there are notable exceptions. VHI can contribute to financial protection especially where it plays a substitutive and complementary role covering co-payments. However it is a complex challenging and highly context-specific policy instrument that may undermine other health system goals including equitable access efficiency transparency and accountability even where markets are well regulated. Policy-makers should therefore exercise real caution before expanding VHI to fill coverage gaps. This volume and its companion set of country profiles were developed jointly by the Observatory s LSE hub and the WHO Regional Office for Europe. The study draws on contributions from national experts from the EU EFTA and other countries in Europe.

This report focuses on the role of voluntary health agencies in support of medical and health-related research and development. Health voluntaries are associations which solicit financial support from the general public to carry on activities which benefit the health of the community. Such program activities include medical and health-related R&D, health education, and medical services activities. Voluntary health agencies are among the organizations which meet the tax exemption standards of Section 501(C)(3) of the Internal Revenue Code, allowing them a status as nonprofit organizations which operate for charitable, scientific, religious, or educational purposes. To the layman, voluntary health agencies are charities. The study reported here provides a profile of Voluntary Health Agency support of health R&D. The early sections provide descriptions of several major features of such support. The last section summarizes a test of the feasibility of using regression analysis for estimating Voluntaries' expenditures for years in the immediate future. The study was written in the early months of 1975 before final figures for 1974 expenditures of the health agencies were available. Projections, therefore, focus on 1974 and 1975.
