It should be noted that in this thesis the term 'integrated health system' is used to cover the full range of varying degrees of integration of traditional with modern medicine and should not be taken to imply only a fully integrated system. Similarly, the terms 'integration' and 'incorporation' are normally used to refer to the process and not the outcome. Even an incorporated health system may not be a viable proposition in all parts of PNG. Where it is feasible, incorporation would need to be progressed in a carefully considered and planned manner with a realistic and long-term approach. The process would require coordination at national level and the flexibility for provinces to participate according to their own prevailing circumstances and capacity. Incorporation should proceed slowly and will require government support including the allocation of resources. It may be possible to pilot and thus fine-tune PNG's integration model in a few places, such as the Nasioi area, before expanding to multiple provinces. The potential benefits of an incorporated health system include strengthening of primary health care, better access to services, more affordable services, cultural relevance, a holistic approach, preservation of traditional knowledge, increased autonomy and possibly cost savings. An incorporated health system is worth pursuing because, if carefully planned and implemented, it does have the potential to improve health status in a country where health indicators desperately need to be elevated.

Questions about the origins of human cooperation have long puzzled and divided scientists. Social norms that foster fair-minded behavior, altruism and collective action undergird the foundations of large-scale human societies, but we know little about how these norms develop or spread, or why the intensity and breadth of human cooperation varies among different populations. What is the connection between social norms that encourage fair dealing and economic growth? How are these social norms related to the emergence of centralized institutions? Informed by a pioneering set of cross-cultural data, Experimenting with Social Norms advances our understanding of the evolution of human cooperation and the expansion of complex societies. Editors Jean Ensminger and Joseph Henrich present evidence from an exciting collaboration between anthropologists and economists. Using experimental economics games, researchers examined levels of fairness, cooperation, and norms for punishing those who violate expectations of equality across a diverse swath of societies, from hunter-gatherers in Tanzania to a small town in rural Missouri. These experiments tested individuals’ willingness to conduct mutually beneficial transactions with strangers that reap rewards only at the expense of taking a risk on the cooperation of others. The results show a robust relationship between exposure to market economies and social norms that benefit the group over narrow economic self-interest. Levels of fairness and generosity are generally higher among individuals in communities with more integrated markets. Religion also plays a powerful role. Individuals practicing either Islam or Christianity exhibited a stronger sense of fairness, possibly because religions with high moralizing deities, equipped with ample powers to reward and punish, encourage greater prosociality. The size of the settlement also had an impact. People in larger communities were more willing to punish unfairness compared to those in smaller societies. Taken together, the volume supports the hypothesis that social norms evolved over thousands of years to allow strangers in more complex and large settlements to coexist, trade and prosper. Innovative and ambitious, Experimenting with Social Norms synthesizes an unprecedented analysis of social behavior from an immense range of human societies. From the President of the Research Society on Alcoholism In the last decade research concerning the causes and consequences of alcohol abuse and alcoholism has come of age. We have witnessed a plethora of sci entific findings that have shed light on some of the actions of alcohol at the molecular level. Interesting new data have been forthcoming on the complexi ties of the development of tolerance to alcohol. It is becoming increasingly appropriate to consider that tolerance to alcohol involves biological as well as psychological factors. New scientific insights have been gained concerning the treatment of with drawal as well as the presence of persistent withdrawal signs that may possibly be involved with relapse. More recently, new and compelling data indicating that alcoholism is a common familial disorder have appeared. Clinical studies indicate that alcoholism is a heterogeneous disorder with multiformity in clin ical symptomatology and genetic heterogeneity. The heterogeneity of the clin ical features and the heritability of the predisposing factors of alcoholism are currently under vigorous scientific investigation. In the past several years sophisticated psychosocial studies have provided fundamental information on subjects at high risk for alcoholism. Psychosocial and biological studies of families including alcoholics and subjects at high risk are likely to bring new insights to our understanding of etiological factors. Moreover, as a result of these studies we stand to develop better prevention initiatives and treatment approaches.
