How can we identify the causes of events? What does it mean to assert that someone is responsible for a moral affront? Under what circumstances should we blame others for wrongdoing? The related, but conceptually distinct, issues of causality, responsibility, and blameworthiness that are the subject of this book play a critical role in our everyday social encounters. As very young children we learn to assert that "it wasn't my fault," or that "I didn't mean to do it." Responsibility and blame follow us into adulthood, as personal or organizational failings require explanation. Although judgments of moral accountability are quickly made and adamantly defended, the process leading to those judgments is not as simple as it might seem. Psychological research on causality and responsibility has not taken complete advantage of a long tradition of philosophical analysis of these concepts. Philosophical discussions, for their part, have not been sufficiently I1ware of the psychological realities. An assignment of blame is a social explanation. It is the outcome of a process that begins with an event having negative consequences, involves judgments about causality, personal responsibility, and possible mitigation. The result can be an assertion, or a denial, of individual blameworthiness. The purpose of this book is to develop a comprehensive theory of how people assign blame.

Few studies have examined perceptions of responsibility attribution and well-being, and health responsibility behaviors in individuals with none, one, and multiple chronic illnesses. Adults (N = 421) in medical clinics completed questionnaires for this cross-sectional descriptive study. The Revised Causal Dimensional Scale measured responsibility attribution whereas sub-sections of the Chronic Disease Self-Management Program evaluation measured well-being perception and the health responsibility behavior of communication associated with a clinic visit. Chi-square, Wilcoxon-Mann-Whitney U, and Kruskal-Wallace tests were used to examine demographic characteristics and group differences. Individuals with multiple morbidities and of advanced age perceived their illnesses as more stable and rated their well-being lowest. Females, participants of advanced age, and living with a partner engaged more in health communication. More information about responsibility attribution and chronic illness management is needed. Study results suggest regular assessment of well-being perception and communication training for individuals >45 years to support multiple chronic illness management.

Medicine is a complex social institution which includes biomedical research, clinical practice, and the administration and organization of health care delivery. As such, it is amenable to analysis from a number of disciplines and directions. The present volume is composed of revised papers on the theme of "Responsibility in Health Care" presented at the Eleventh Trans Disciplinary Symposium on Philosophy and Medicine, which was held in Springfield, illinois on March 16-18, 1981. The collective focus of these essays is the clinical practice of medicine and the themes and issues related to questions of responsibility in that setting. Responsibility has three related dimensions which make it a suitable theme for an inquiry into clinical medicine: (a) an external dimension in legal and political analysis in which the State imposes penalties on individuals and groups and in which officials and governments are held accountable for policies; (b) an internal dimension in moral and ethical analysis in which individuals take into account the consequences of their actions and the criteria which bear upon their choices; and (c) a comprehensive dimension in social and cultural analysis in which values are ordered in the structure of a civilization ([8], p. 5). In August, 1985, the 2nd International Conference on Illness Behaviour was held in Toronto, Ontario, Canada. The first International Conference took place one year previous in Adelaide, South Australia, Australia. This book is based on the proceedings of the second conference. The purpose behind this conference was to facilitate the development of a single integrated model to account for illness experience and presentation. A major focus of the conference was to outline methodological issues related to current behaviour research. A multidiscipl~nary approach was emphasized because of the bias that collaborative efforts are likely to be the most successful in achieving greater understanding of illness behaviour. Significant advances in our knowledge are occurring in all areas of the biological and social sciences, albeit more slowly in the latter areas. Marked specialization in each of these areas has lead to greater difficulty in integrating new knowledge with that of other areas and the development of a meaningful cohesive model to which all can relate. Thus there is a major need for forums such as that provided by this conference.
