European public health was a playing field for deeply contradictory impulses throughout the twentieth century. In the 1920s, international agencies were established with great fanfare and postwar optimism to serve as the watchtower of health the world over. Within less than a decade, local-level institutions began to emerge as seats of innovation, initiative, and expertise. But there was continual counterpressure from nation-states that jealously guarded their policymaking prerogatives in the face of the push for cross-national standardization and the emergence of original initiatives from below. In contrast to histories of twentieth-century public health that focus exclusively on the local, national, or international levels, Shifting Boundaries explores the connections or "zones of contact" between the three levels. The interpretive essays, written by distinguished historians of public health and medicine, focus on four topics: the oscillation between governmental and nongovernmental agencies as sites of responsibility for addressing public health problems; the harmonization of nation-states' agendas with those of international agencies; the development by public health experts of knowledge that is both placeless and respectful of place; and the transportability of model solutions across borders. This work resulted from a conference held in 2003 that was jointly sponsored by the Rockefeller Archive Center and Quinnipiac University. Drawing upon perspectives from history, philosophy, and the social sciences, as well as public health and medicine, the authors in this volume examine and critique the role of Foundations, most prominently the Rockefeller Foundation, in promoting and expanding the development of Western medicine around the world during the 20th century. The first half of the book examines the historical involvement of philanthropic foundations in public heath, basic medical research, and related social and political issues. These studies range from an examination of the Rockefeller's Foundation's anti-malaria campaigns to the involvement of Foundations in promoting eugenic ideology and population control. The second half of the book considers current situations in which philanthropic foundations are active in promoting public health and westernized medicine, including consideration of the fight against AIDS in Africa, the resurgence of tuberculosis as a major public health threat, and the ongoing war against malaria. Present trends indicate that in the years to come transnational science, whether basic or applied and involving persons, equipment or funding, will grow considerably. The main purpose of this volume is to try to understand the reasons for this denationalization of science, its historical contexts and its social forms. The Introduction to the volume sets out the socio-political, intellectual, and economic contexts for the nationalization and denationalization of the sciences, processes that have extended over four centuries. The articles examine the specific conditions that have given rise to the growth of transnational science in the 20th century. Among these are: the need for cognitive and technical standardization of scientific knowledge-products, pressure toward cost-sharing of large installations such as CERN, the voluntary and involuntary migration of scientists, and the global market for R&D products that has emerged at the end of the century. The volume raises many new questions for research by historians and sociologists of science and poses problems that are of concern both to scientists and science policy-makers.
