Introduction : l'infertilité est un enjeu de santé publique, et le recours à la procréation médicalement assistée (PMA) augmente. Le nombre de gynécologues médicaux diminue en France alors que la loi de bioéthique de 2021 élargit l'accès à la PMA. Le médecin généraliste de premier recours a donc un rôle à jouer dans la prise en charge de l'infertilité. Objectif : déterminer le(s) rôle(s) du médecin généraliste dans le parcours de soins en PMA et à quelle étape il peut intervenir. Méthode : étude quantitative observationnelle par questionnaire papier anonyme auprès des couples hétérosexuels, couples de femmes et femmes célibataires consultant au centre de fertilité de la clinique d'Occitanie à Muret. Résultats : 78 réponses ont été obtenues, soit un taux de réponse de 30,7%. 65,4% des participants pensaient que le médecin généraliste devait être impliqué dans la prise en charge de l'infertilité. Plusieurs rôles ont été proposés avant, pendant et après la prise en charge en centre spécialisé. Le rôle principal a été l'orientation vers le spécialiste. Discussion et conclusion : le médecin généraliste a un rôle essentiel dans la prévention et la prise en charge des facteurs de risque d'infertilité avec un rôle d'éducation en santé, dans le cadre du suivi pré-conceptionnel des hommes et des femmes. Le soutien psychologique doit être proposé à tous les couples infertiles. L'amélioration de la formation initiale et l'approfondissement de la formation continue sur le sujet sont nécessaires à une prise en charge optimale en accord avec les dernières recommandations.

Introduction : sous la pression de mouvements conservateurs religieux notamment, certains pays où l'avortement est légal, sont prêts à remettre en question le droit des femmes à disposer de leur corps à l'instar des États-Unis en mai 2022. Qu'en est-il de l'accessibilité de l'IVG à travers le monde ? Et de la place du médecin généraliste dans le parcours de soins ? Méthode : nous avons mené une revue systématique de la littérature. Résultats : en Europe, seuls deux états, le Vatican et Andorre, n'autorisent pas l'IVG. Le médecin généraliste n'intervient que très peu dans la prescription de l'avortement médicamenteux, mise à part en France, en Belgique et en Irlande. Il agit surtout en prévention primaire par la prescription de la contraception et le suivi des patientes à risque. Hors des frontières européennes, il existe une grande disparité concernant le droit et l'accès à l'avortement. Il est soit autorisé pour sauver la vie de la femme, sa santé physique ou mentale, pour des raisons économiques ou à la demande mais est toujours restreint à un certain nombre de semaines d'aménorrhée. Ailleurs, le médecin généraliste est peu sollicité pour la pratique des IVG médicamenteux sauf en Australie. L'avortement est pratiqué par toute femme, même dans les pays où il est illégal, et par tout moyen (méthode chimique, mécanique, médecine traditionnelle, régulation menstruelle). Des marchés parallèles se développent, avec la délivrance de misoprostol par les pharmaciens. Les avortements illégaux mettent en danger la vie des patientes qui y ont recours et accroissent la mortalité des femmes dans les pays du Tiers Monde. Pour pallier l'interdiction à l'avortement, on met en place dans certains pays des soins post-avortement, renforçant finalement d'avantage les inégalités en matière de reproduction. Malheureusement, là où il est légal, on observe des dérives avec la pratique de l'avortement sexo-sélectif comme en Asie, et une accessibilité encore difficile par manque de personnel qualifié. Conclusion : toute femme dans le monde n'est pas égale quant à son accès à l'IVG. Le médecin traitant agit peu dans le parcours de soins de la femme, son rôle est à développer dans la prescription des avortements médicamenteux et en matière de prévention primaire par la contraception et le suivi des populations à risque.

Genetics is in all senses a family affair. The diagnosis of a genetic condition affects not only the patient and biological family members who may themselves be at risk, but also "family" more generally as support may be sought from those considered kin and who may or may not be at risk themselves. It is considered best practice in genetic consultations to explore who will be informed within the family when a genetic condition or risk is diagnosed, particularly when the health of other family members is at risk. There is little guidance or consensus on how to achieve the implicit goal of informed family members while respecting patient confidentiality, however. There is a need for practitioners to be aware of the dynamics of family communication and to have guidance about how they may sensitively facilitate communication about genetics within families. This handbook facilitates the development of clinical practices relating to family communication about genetics. Relevant theories of family communication are summarized and related to a clinical genetics milieu and, from this, frameworks for practitioners are presented. A book of this nature is particularly timely as the completion of the Human Genome Project will result in an unprecedented amount of information about genetic constitution and health risks becoming available to individuals and their families. The presence of a potentially genetic condition in a family is not a new phenomenon. However, the growth in testing for genetic conditions, common complex conditions and variants that may influence health as well as drug metabolism means that a greater number of individuals will face decisions about communicating this information to their relatives.
