Contexte : L'élaboration de guides de bonne pratique, l'amélioration de l'environnement sanitaire et l'évolution du cadre législatif ont permis de constituer au fur et à mesure un ensemble de normes encadrant les soins prodigués à la personnes malade en fin de vie et l'accompagnement dont elle peut bénéficier. Pourtant si la majorité des français en bonne santé déclarent vouloir mourir à domicile, un grand nombre d'entre eux décède à l'hôpital. Quant est-il à l'échelle du département de la Loire et pourquoi un tel constat? Objectifs :- Etudier les lieux de décès dans la Loire. - Montrer l'influence de la fragilité du patient en fin de vie hospitalisé sur son retour à domicile. Méthode : Nous avons réalisé deux études : - une exhaustive des certificats de décès des patients morts dans le département de la Loire en janvier 2006. - une épidémiologique d'observation, prospective, longitudinale, multicentrique, incluant des cas consécutifs de patients hospitalisé en fin de vie souhaitant retourner à domicile. Résultats : Notre état des lieux des décès dans la Loire confirme une nette majorité des décès (60%) à l'hôpital. De plus les décès à domicile correspondent le plus souvent à des décès brutaux. L'étude du retour à domicile des personnes en fin de vie montre deux principaux résultats : d'une part une proportion importante (56%) de patients qui le souhaitait, a pu rentrer « à la maison », d'autre part le niveau de fragilité du patient en fin de vie, défini par des facteurs cliniques, éthiques et pratiques est un facteur prédictif de retour à domicile. Synthèse : La fragilité des patients peut donc expliquer le retour ou non à domicile des patients, cependant la connaissance des soignants de leurs patients est également un élément clef.

Nostradamus (Michel de Nostradame) was born on December 14, 1503 in St. Remy, Provence, France. Nostradamus came from a long line of Jewish doctors and scholars. He is considered by many as one of the most famous and important writers of history prophecies. He is famous mainly for his book 'The Prophecies, ' consisting of quarantine in rhyme. Supporters of the trustworthiness of these prophecies attribute to Nostradamus the ability to predict an incredible number of events in world history, including the French Revolution, the Atomic bomb, the rise to power of Adolf Hitler and the attacks of 11 September 2001. However, no one has ever proved that Nostradamus's quarters can provide reliable data for the foreseeable future. Nostradamus had the visions which he later recorded in verse while staring into water or flame late at night, sometimes aided by herbal stimulants, while sitting on a brass tripod. The resulting quatrains (four line verses) are oblique and elliptical, and use puns, anagrams and allegorical imagery. Most of the quatrains are open to multiple interpretations, and some make no sense whatsoever. Some of them are chilling, literal descriptions of events, giving specific or near-specific names, geographic locations, astrological configurations, and sometimes actual dates. It is this quality of both vagueness and specificity which allows each new generation to reinterpret Nostradamus.
