L'importance de l'adaptation du discours du médecin et de sa pratique à chaque patient est indispensable. Cette nécessité et le fort pourcentage de consultations de pédiatrie en médecine de soins primaires nous ont poussés à nous interroger sur les représentations que l'enfant de 5-6 ans, avec sa façon originale d'interpréter le monde qui l'entoure, se fait des consultations qu'il a auprès de son médecin généraliste. Au travers d'entretiens collectifs au sein d'une classe de grande section d'école maternelle de 15 élèves, puis de représentations graphiques individuelles, nous avons donc cherché à savoir ce que les enfants pouvaient dire de leur recours au médecin, du lieu de la consultation, de ce qu'il s'y passe, de leurs attentes, et du médecin qu'ils rencontrent en consultation. Pour les enfants la consultation n'est pas systématique et nécessite l'élaboration d'une démarche. Ils attendent d'être soulagés, de savoir ce qu'ils ont et accordent de l'intérêt à la santé de leurs proches. Ils soulignent l'importance de la salle d'attente et de ses jeux. Ils invitent le médecin à entendre, tout en l'encadrant, leur parole, à être attentif à la sexualité infantile, à contribuer au développement de leur estime de soi, à ne pas nier leur subjectivité. Au travers de cette étude, les enfants nous rappellent leur particularité d' “êtres en pleine construction identitaire”, construction dont le médecin, par les fonctions symboliques qu'il incarne, est un acteur important. En proposant au médecin généraliste de voir la consultation avec les yeux de l'enfant de 5-6 ans, notre travail pourrait lui permettre de mieux le comprendre et d'optimiser la relation de soins.

Introduction : Les consultations pédiatriques occupent une place importante dans l'exercice de la médecine générale. Elles sont caractérisées par la complexité liée à la relation tridimensionnelle avec l'enfant et ses parents. Une enquête visant à mieux comprendre quelle image les enfants ont de leur médecin généraliste a été menée, dans la perspective de mieux déchiffrer les caractéristiques de la relation médecin-patient enfant, pour contribuer à l'amélioration de la qualité des soins. Matériel et Méthode : Un recueil de dessins d'enfants âgés de 6 à 9 ans ayant reçu la consigne : « Dessine ton docteur », a été réalisé dans des écoles primaires de Loire-Atlantique. Une analyse descriptive en a été faite. Résultats : L'image du médecin semble avoir évolué. Ce personnage est perçu aujourd'hui par la majorité des enfants de l'enquête comme accueillant, sympathique et accessible. Les dessins témoignent de la possibilité de construire une relation individuelle et personnalisée avec le médecin. Mais pour la majorité des enfants de l'étude, la consultation médicale peut être source d'angoisse. Pour quelques garçons, l'angoisse semble extrême ; à l'opposé, un certain nombre de filles paraissent complètement à l'aise et en sécurité. Conclusion : Comprendre comment les enfants perçoivent le médecin et comment ils vivent une consultation médicale peut sans doute aider à construire une relation de qualité avec eux. Par les informations qu'il délivre sur le monde intérieur des jeunes patients, le dessin d'enfant peut être un outil extrêmement intéressant en consultation pour les médecins généralistes désireux de pénétrer la psychologie enfantine.

Ageing populations have gradually become a major concern in many industrialised countries over the past fifty years, drawing the attention of both politics and science. The target of a raft of health and social policies, older people are often identified as a specific, and vulnerable, population.
