We discuss regional disparities in economic performance and living standards. We first set out some key facts, and provide a conceptual framework to help analyze whether such disparities are efficient, or instead reflect market and/or policy failures. We examine whether policy attempts to reduce regional disparities necessarily involve a trade-off between equity and efficiency. We then investigate whether policymakers should focus on boosting the economic performance of lagging regions—or, conversely, accept the presence of regional disparities, and instead assist households in lagging regions through transfer payments, investments in education, health, and other basic services, and by facilitating out-migration.

Based on eight monographs presented and discussed at a working meeting of the International Institute for Educational Planning (IIEP) in 1977, this summational report is intended to be used by educational planners for a reduction of regional disparities through school network reorganization and improvements in educational service. Part I discusses the regional approach to the problem of educational disparities with reference to countries such as Thailand and Kenya, identifying both interregional and intraregional disparities. Part II establishes criteria for measuring disparities, beginning with such commonly used indicators as quantitative expansion of the education system and efficiency of educational service, and demonstrating three different types of evaluative analysis: evolution of disparities in time; evolution of disparities at different levels of the education system; and the relation between educational, economic, and social disparities. Part II also includes a proposed classification of educational indicators for analytical purposes. While part III focuses on disparities in educational access, school progress, and scholastic achievement, part IV reviews measures currently envisaged by governments for reducing regional disparities and discusses their implications for educational planning. In the United States, some populations suffer from far greater disparities in health than others. Those disparities are caused not only by fundamental differences in health status across segments of the population, but also because of inequities in factors that impact health status, so-called determinants of health. Only part of an individual's health status depends on his or her behavior and choice; community-wide problems like poverty, unemployment, poor education, inadequate housing, poor public transportation, interpersonal violence, and decaying neighborhoods also contribute to health inequities, as well as the historic and ongoing interplay of structures, policies, and norms that shape lives. When these factors are not optimal in a community, it does not mean they are intractable: such inequities can be mitigated by social policies that can shape health in powerful ways. Communities in Action: Pathways to Health Equity seeks to delineate the causes of and the solutions to health inequities in the United States. Four lengthy papers and an introduction discuss case studies of regional disparities in educational development in Cameroon, Madagascar, Kenya, Tanzania, and Thailand. In considering the specific political and socioeconomic contexts of these countries, the papers focus on methods of both analyzing the causes of disparities and reducing the resultant inequalities. The introduction by the two editors summarizes the papers and the issues involved. Jean-Yves Martin's paper on Cameroon and Philippe Hugon's on Madagascar examine the sociohistorical causes of regional disparities and discuss the proper geographical unit to use in measuring educational disparities. Kamol Sudaprasert, Vichai Tunsiri, and Ta Ngoc Chau address the problem of data analysis in a paper on Thailand's regional disparities in educational access, social characteristics, and socioeconomic development. Finally, David Court and Kabiru Kinyanjui compare Kenyan and Tanzanian efforts to reduce regional disparities: Kenya attempted to expand its educational system while Tanzania tried to redistribute educational opportunities. In early 2007, the Institute of Medicine convened the Roundtable on Health Disparities to increase the visibility of racial and ethnic health disparities as a national problem, to further the development of programs and strategies to reduce disparities, to foster the emergence of leadership on this issue, and to track promising activities and developments in health care that could lead to dramatically reducing or eliminating disparities. The Roundtable's first workshop, Challenges and Successes in Reducing Health Disparities, was held in St. Louis, Missouri, on July 31, 2007, and examined (1) the importance of differences in life expectancy within the United States, (2) the reasons for those differences, and (3) the implications of this information for programs and policy makers.
