INTRODUCTION : Le sepsis sévère et le choc septique sont des motifs d'admission fréquents avec une forte mortalité associée. Une diminution a été observée grâce à la Surviving Spesis Campaign (SCC). Le but de notre travail a été d'analyser l'état des connaissances des internes aux urgences sur la prise en charge du sepsis sévère versus les recommandations actuelles. METHODOLOGIE : C'est une étude observationnelle élaborée grâce à un questionnaire destiné aux internes de médecine générale de Marseille et de Nancy. Il a été diffusé du 22 avril 2013 ua 22 juin 2013. RESULTATS : 69 internes sur 982 y ont répondu. De bonnes réponses ont été obtenues concernant : les hémocultures, le délai d'instauration des antibiotiques, les cristalloïdes, la Narodrénaline, la glycémie, la prophylaxie des thromboses veineuses profondes, l'entretient avec les familles. DE moins bons résultats ont été constatés concernant : les lactates, le monitorage, le dosage du soluté de remplissage, les objectifs de Pression Artérielle Moyenne et d'Hémoglobine, la prophylaxie de l'ulcère de stress, l'alimentation et la connaissance de l'existence de la SCC. CONCLUSION : Malgré un faible taux de participation, l'étude a montré que les internes avaient des connaissances importantes sur certains aspects fondamentaux des recommandations actuelles mais qu'ils manquaient de données quant au monitorage, à l'existence même de la SCC et de connaissances par rapport aux protocoles

It constitutes a unique source of knowledge and guidance for all healthcare workers who care for patients with sepsis and septic shock in resource-limited settings. More than eighty percent of the worldwide deaths related to sepsis occur in resource-limited settings in low and middle-income countries. Current international sepsis guidelines cannot be implemented without adaptations towards these settings, mainly because of the difference in local resources and a different spectrum of infectious diseases causing sepsis. This prompted members of the Global Intensive Care working group of the European Society of Intensive Care Medicine (ESICM) and the Mahidol-Oxford Tropical Medicine Research Unit (MORU, Bangkok, Thailand) - among which the Editors – to develop with an international group of experts a comprehensive set of recommendations for the management of sepsis in resource-limited settings. Recommendations are based on both current scientific evidence and clinical experience of clinicians working in resource-limited settings. The book includes an overview chapter outlining the current challenges and future directions of sepsis management as well as general recommendations on the structure and organization of intensive care services in resource-limited settings. Specific recommendations on the recognition and management of patients with sepsis and septic shock in these settings are grouped into seven chapters.
