Ten years after the discovery of prostacyclin, our knowledge of its biochemical mode of action, pharmacological properties, pathophysiological significance and therapeutic applications is ever expanding. Prostacyclin is both complex and unique as demonstrated by its unusual feature of being chemically and meta bolically unstable when compared to other prostanoids and known amine or peptide mediators. Although physiologically essential, the chemical instability of prostacyclin poses a serious drawback in laboratory and clinical studies. It is one of the genuine objectives of pharmaceutical research to supply synthetic compounds which overcome the inherent drawbacks - considering investigational and therapeutic use - of endogenous compounds. Whereas metabolic instability in certain cases could be of advantage, chemical instability definitely is not. With Iloprost, a molecule has been designed which - according to all data so far available - pertains high receptor affinity, metabolic instability (clinically this equates with a fine control of Iloprost's effects) while chemical stability has been achieved. By virtue of these characteristics Iloprost can be considered as one step towards specific interaction within the arachidonic acid cascade, namely the prostacyclin receptor. The aim of the symposium was to provide a critical experimental appraisal concerning the biochemical mode of action and pharmacological properties of prostacyclin and Iloprost.

Arterial hypertension, coronary heart disease and heart fail ure are the commonest cardiovascular conditions to present in clinical practice. Over the past few years it has become in creasingly clear that they are closely and causally interrelated and that their relationship can have a significant bearing on prognosis. Epidemiological studies have shown that arterial hypertension is one of the most important risk factors for de veloping heart failure. Only one in four patients with hyper tension is adequately managed, and in 50% of cases, the hypertension has not been recognised or treated. Patients with pre-existing hypertension who go on to suffer an acute myocardial infarction have usually not previously had typi cal angina symptoms, the infarct territory is larger, life threatening arrhythmias are commoner and hence in-hospi tal mortality and long-term prognosis are markedly worse. The presence of raised blood pressure in the post-infarct phase doubles the risk of manifest heart failure. The close relationship between hypertension, coronary heart disease and heart failure makes the choice of therapeu tic strategy particularly important. Agents and classes of agents that have prognostic value in all three conditions should be considered first, as synergy might result in addi tional benefits. In such patients, this sort of therapeutic deci sion-making might have further advantages. The use of these agents may prevent complications which are not yet clinically obvious (such as heart failure).

The endothelium, a monolayer of endothelial cells, constitutes the inner cellular lining of the blood vessels (arteries, veins and capillaries) and the lymphatic system, and therefore is in direct contact with the blood/lymph and the circulating cells. The endothelium is a major player in the control of blood fluidity, platelet aggregation and vascular tone, a major actor in the regulation of immunology, inflammation and angiogenesis, and an important metabolizing and an endocrine organ. Endothelial cells controls vascular tone, and thereby blood flow, by synthesizing and releasing relaxing and contracting factors such as nitric oxide, metabolites of arachidonic acid via the cyclooxygenases, lipoxygenases and cytochrome P450 pathways, various peptides (endothelin, urotensin, CNP, adrenomedullin, etc.), adenosine, purines, reactive oxygen species and so on. Additionally, endothelial ectoenzymes are required steps in the generation of vasoactive hormones such as angiotensin II. The ultrasound velocity tomography allows measurement of cardiac geometries for various phases in the cardiac cycle. The present tomograph makes reconstruc tions at intervals of 20 ms. Because of a lack of clear (intramural) landmarks (except the roots of the papillairy muscle), it is difficult to pinpoint spatial trajectories of particular points in the heart. Therefore, a second method was developed of injecting radiopaque markers in the heart and following their motion patterns during the cardiac cycle with help of a biplane X-ray equipment. The data obtained with both methods can be implemented in our finite element model of the heart to compute intramural stresses and strains. The results obtained sofar with the extended Darcy equation to account for the interaction of blood rheology and tissue mechanics look promising. Further testing with more sophisticated subjects than mentioned in Figure 9 is required before it will be implemented in our finite element model of the heart. We conclude that analysis of regional cardiac function, including regional myocardial blood flow, requires still a major research effort but the results obtained sofar justify, to our opinion, a continuation in this direction.
