Contexte : En 2012, les services d'urgences du Nord-Pas-de-Calais ont enregistré 1 117 543 passages, 5,3% concernait une affection de l'appareil respiratoire. Les malades consultant pour un bronchospasme sont particulièrement fragiles. Un retard de prise en charge peut mettre en jeu leur pronostic vital. Le terrain sur lequel survient le bronchospasme conditionne la prise en charge. Les sociétés savantes fournissent des recommandations de prise en charge pour ces étiologies difficiles. En pratique, sont-elles suivies? Méthode : Etude prospective, observationnelle, descriptive, multicentrique, réalisée aux urgences des centres hospitaliers de Douai et Roubaix entre le 12 Février et le 12 Mars 2015. Tous les adultes admis en service d'urgences, présentant une dyspnée expiratoire associée à des sibilants auscultatoires, quel que soit le terrain ou la gravité, étaient inclus dans l'étude. Les praticiens renseignaient sur un questionnaire à choix multiples les caractéristiques du patient, le diagnostic et leur prise en charge thérapeutique. Cette dernière était comparée aux recommandations émises par les sociétés savantes. Résultats : 67 patients ont été inclus, 28 au centre hospitalier de Douai et 39 au centre hospitalier de Roubaix, dont 29 femmes. L'âge moyen des patients était de 62,8 ans ; écarttype 19,8. Les étiologies reconnues comme responsables du bronchospasme aigu ont été une exacerbation de broncho-pneumopathie chronique obstructive dans 46% des cas, 21% un asthme aigu grave. Sur les 67 diagnostics évoqués, 48 faisaient l'objet de recommandations par les sociétés savantes. Les recommandations de prise en charge ont été suivies dans 63% des cas (n=30). Conclusion : Les recommandations de prise en charge des bronchospasmes, quand elles existent, ne sont suivies que dans 63% des cas, ce qui signifie que plus d'un patient sur 3 se présentant aux urgences pour ce motif ne bénéficie pas d'une prise en charge optimale.

Childen's palliative care has developed rapidly as a discipline, as health care professionals recognize that the principles of adult palliative care may not always be applicable to children at the end of life.
