Sélection d'une population d'adolescentes de 15 à 19 ans scolarisées en lycée d'enseignement professionnel et général. Résultats : il persiste un manque de connaissance sur la contraception. Même après avoir rencontré un professionnel, les adolescentes ne retiennent pas l'étendue de l'offre et donc peuvent difficilement faire un choix. La contraception orale a été prescrite en première prescription pour toutes les adolescentes alors qu'elle ne semble pas convenir à toutes. Conclusion : de nombreux enjeux émanent de cette première prescription, il est donc important pour nous professionnels, de s'assurer de l'exhaustivité des informations retenues pour que la contraception soit adaptée et acceptée par l'adolescente. Une contraception choisie sera plus efficace qu'une contraception imposée non prise, et ce même si son efficacité théorique est initialement plus faible.

La réalisation d’une étude qualitative, à l’aide d’une enquête par 10 entretiens semi-dirigés de médecins généralistes de la Somme installés en libéral, a permis de comprendre leur démarche. La prescription initiale de contraception va au-delà des critères médicaux. Même si l’examen médical est une étape incontournable, le médecin fait face à des obstacles. Il doit s’adapter à une relation triangulaire avec la maman révélant un biais de confidentialité et du motif réel de la demande. Il doit orienter son discours selon l’âge de la personne, son contexte socio-culturel, sa motivation et sa rigueur. Le facteur économique est prépondérant mais le défaut de connaissance est aussi important. Il doit délivrer une information idéalement complète sur la contraception, la contraception d’urgence et les IST. Pas simple en pratique, la démarche éducative reste peu utilisée et le manque de temps est réel. Il doit tenter de rendre l’adolescente autonome dans ses choix pour l’amener vers une contraception adaptée à ses besoins. Les autres formes de contraception que la pilule restent peu abordées, les croyances biaisant leur utilisation. Les connaissances du médecin sont fondées sur son expérience professionnelle et son vécu personnel, la formation initiale étant manquante la plupart du temps. Il faut poursuivre les efforts de remboursement, d’information à grande échelle, d’information par le médecin et le développement des connaissances des médecins.
