Treatment of dyslipidemia in HIV-infected persons is commonly required with the use of protease inhibitors (PIs) which may be restricted by drug-drug interactions between antiretroviral agents and statins. We hypothesized that darunavir/ritonavir (DRV/rtv) modulate the pharmacokinetics of rosuvastatin (RSV) when co-administered by altering its hepatic disposition. The plausible genetic influence on the plasma exposure of RSV was also explored. To investigate this hypothesis, we first conducted a clinical drug interaction study and subsequently performed in vitro correlative/mechanistic analysis. In the open label, crossover study, 12 healthy volunteers were randomized to receive either RSV 10 mg/day or DRV/rtv 600/100 mg (bid) and in combination for 7 days followed by a crossover to the other regimen for 7 more days. The fasting lipids were obtained at baseline and on days 7, 21 and 35 along with intensive PK sampling for 24 hours post-dose. The PK parameters such as Cmax, t1/2 and AUC0-24 were determined by non-compartmental analysis using WinNonLin 5.2. Statistical analyses were performed using Paired t test. Drug levels were compared with OATP1B1 c.521T> C, 388A>G, BCRP c.421 C> A, MRP2 c.-24C>T, 1249G>A, 3972C>T single nucleotide polymorphisms (SNPs) to investigate potential outliers in the drug interaction study. Next we assessed the influence of DRV on the activity of multidrug transporters OATP1B1, BCRP and Pgp in the transporter over-expressing CHO and MDCK cells and primary human hepatocyte cultures. Complex antiretroviral therapy has introduced new obstacles in the treatment of HIV infection such as management of drug-induced toxicities and drug-drug interactions, which are often attributed to changes in cytochrome P450 (CYP)-mediated metabolism. However, interactions between drugs that do not share the same metabolic pathway, such as HIV protease inhibitors (PIs) and nucleoside/nucleotide reverse transcriptase inhibitors (NRTIs), remain unexplained. In addition, some PIs (e.g., atazanavir) have pH dependent oral bioavailability and interact with acid-reducing agents, through an unknown mechanism that may involve increased gastrointestinal pH. Human intestinal epithelium expresses a number of ATP-binding cassette transporters (e.g., P-glycoprotein) and solute carrier transporters (e.g., organic anion transporting polypeptides (OATPs)), which can restrict or facilitate intestinal drug absorption. Over the years a number of excellent books have classified and detailed drug drug interactions into their respective categories, e.g. interactions at plasma protein binding sites; those altering intestinal absorption or bioavailability; those involving hepatic metabolising enzymes; those involving competition or antagonism for receptor sites, and drug interactions modifying excretory mechanisms. Such books have presented extensive tables of interactions and their management. Although of considerable value to clinicians, such publica tions have not, however, been so expressive about the individual mechanisms that underlie these interactions. It is within this sphere of "mechanisms" that this present volume specialises. It deals with mechanisms of in vitro and in vivo, drug-drug, drug food and drug-herbals interactions and those that cause drugs to interfere with diagnostic laboratory tests. We believe that an explanation of the mechanisms of such interactions will enable practitioners to understand more fully the nature of the interactions and thus enable them to manage better their clinical outcome.
