Objectifs : La loi Leonetti, votée le 22 avril 2005, apporte aux médecins le cadre législatif pour la réflexion dans les situations complexes de soins de fin de vie. Cette étude rétrospective descriptive a pour objectif principal d’identifier les motifs d’hospitalisation en urgence des personnes en fin de vie et d’analyser l’opinion des médecins traitants. Les objectifs secondaires sont l’évaluation des conditions de décès à l’hôpital et de l’application de la loi Leonetti. Méthode : L’étude concerne 37 patients en fin de vie décédés en 2007 à l’hôpital de Chambéry 10 jours suivant leur entrée par les urgences. Cinq médecins tirés au sort parmi les médecins traitants ont été interrogés par entretiens semi-directifs. Leurs propos ont été analysés et un questionnaire envoyé en février 2009 aux 33 médecins traitants. Résultats : 90% des médecins ont répondu au questionnaire. Ils soulignent la difficulté du soutien de la famille (dont l’épuisement est un motif d’hospitalisation fréquemment retrouvé), leur solitude face à la fin de vie et l’insuffisance de concertation avec les médecins hospitaliers. 41% des patients sont décédés dans les 72 premières heures après leur admission et 49% dans un service dont l’équipe soignante ne les connaissait pas. La loi Leonetti n’est connue que par 13% des médecins généralistes et son application à l’hôpital reste insuffisante. Conclusion : Les résultats de cette étude confirment les besoins d’aide aux familles pour favoriser la prise en charge à domicile, de formation des médecins à la communication et à la réflexion éthique mais aussi d’information sur la loi Leonetti.

L'accompagnement des patients en fin de vie au domicile est essentiel pour 70 % des français, selon un sondage de l'IFOP de 2017. La proportion d'hospitalisation double au cours du dernier mois de vie. 44 % des patients décédés en hospitalisation pour soins palliatifs étaient passés par un service d'urgence. Concrètement, comment les soignants (équipe médicale et paramédicale) anticipent-ils les situations d'urgence en fin de vie ? L'objectif de notre étude était de recueillir leurs pratiques sur l'anticipation de ces situations de soins. Une étude qualitative à travers des entretiens semi-dirigés a été réalisée en ambulatoire auprès de deux populations, en Bretagne et en Rhône-Alpes : médecins généralistes d'une part et infirmier.ère.s, aides-soignant.e.s et kinésithérapeutes d'autre part. L'urgence en fin de vie a été définie comme complexe, paradoxale et imprévisible. Les paramédicaux l'ont décrite comme un dépassement de leurs compétences. La mort a pu être évoquée par les médecins comme une urgence. Dans la population paramédicale, les déterminants de l'anticipation étaient liés aux aidants, au patient, au médecin, aux autres soignants ainsi qu'à la situation elle-même. Dans la population médicale, les déterminants étaient médicaux et psychosociaux (intrinsèques et extrinsèques). La place des aidants, les représentations de la fin de vie et le souhait du patient influençaient l'anticipation de tous et toutes les professionnel.le.s. L'interprofessionnalité ressortait comme un pilier des moyens de l'anticipation des situations d'urgence en fin de vie. L'apprentissage par l'expérience et par le partage d'expérience est un point fort de notre étude. A partir de nos résultats, nous avons défini l'urgence en fin de vie comme plurifactorielle : biologique, psychologique et social (de proximité et société). Nous l'avons intégrée dans notre modèle de théorisation basée sur la définition de l'anticipation (A.Villate) comme la rencontre de la prévision, prédiction et prévention. Une application de ce modèle devrait permettre d'améliorer la pratique.
