Research at the molecular and the cellular level has greatly enhanced our understanding of the pathogenesis and management of heart disease. Valuable contributions, towards this end, have been made by scientists from different dis ciplines including biochemistry, physiology, pathology, molecular biology and biophysics. We felt that it would be of interest and value to bring together ex perts from diverse specialities to present their work and to discuss the common problems encountered in their endeavours. In accordance, a symposium was organised in February 1988 at the Postgraduate Institute of Medical Education & Research, Chandigarh. It was held during the annual meeting of the Indian section of the International Society for Heart Research. This book is a compila tion of some of the papers presented at the symposium. The symposium was sponsored by the Council on Cardiac Metabolism of the International Society and Federation of Cardiology. A number of Indian or ganisations gave generous financial help. These included the National Academy of Medical Sciences, Indian Council of Medical Research, Council of Scientific and Industrial Research and Department of Science and Technology. Desktop publishing was used to prepare this volume. In doing so we came to appreciate the remarkable qualities, skills and help rendered by Professor Dharam Vir. The second volume of "Osteologia" - the proceedings of the annual meetings of the German Society for Osteology - differs from the first one in two ways. In contrast to the preceding one this issue is written in English. There had been good reasons for this decision but there had been equally good arguments against it by some board members. We therefore consider this to be a trial. Being aware of the difficulties to prepare a book not in one's mother tongue the editors only made minor corrections of the authors' con tributions. Having published with the first proceedings an overview of the various fields of research on osteology in our country, the conference of the German Society for Osteology now deals with special subjects and so will this conference report. The main heading of this book being "Generalized bone diseases". One chapter will deal with new developments in the field of osteology and another presents unusual cases presented at the meeting. Our hopes are that the efforts of the German Society for Osteology, the local organizers and the editors will find some acknowledgement. First described in 1907 by Schicke but recognized as a clinical entity only as recently as 1958, when Teare published the pathologic findings in patients with hypertrophic cardiomyopathy (HeM), an explosion of knowledge about this fascinating disease has occurred, which has caused a profound evolution of our understanding of its broad pathophysiologic and clinical spectrum. Progress has been particularly rapid in the past few years when M-mode echocardiography, and more recently 2-dimen sional echocardiography have been applied to the study ofHCM. In addition to new insights as to what the disease is, there has been enormous progress concerning its treatment, with the application of beta-adrenergic blocking agents and surgical relief of left ventricular outflow tract obstruction. Although these approaches have led to great strides in the symptomatic control of the disease, many patients' symptoms have remained refactory to medical and surgical therapy. Most discouragingly, sudden death still occurs, even in patients on large doses of beta-blocking agents and in patients who have had surgical relief of left ventricular outflow tract obstruction.
