HIV continues to be a profound challenge facing communities nationally and internationally. Until a vaccine or a cure is found, prevention remains a most crucial line of defense. However, the successes made to reduce exposure and transmission have not benefited all communities equally. HIV continues to affect vulnerable communities, and HIV-related health disparities are growing. Participation has been linked with better health outcomes for young people in a range of settings, with an extensive literature extolling the benefits of a participatory approach to youth-focused HIV-prevention programs in particular. However the processes of participation, and how the ideals outlined in the participation literature can be achieved in the difficult circumstances in which many youth health promotion programs operate, are less often discussed. This thesis responds to calls for more nuanced documentation of situated participatory practices by developing a detailed and contextualised analysis of youth participation in a Photovoice project in the Highlands of Papua New Guinea. The analysis draws upon data generated over a ten-month period (photo-stories, individual interviews, written accounts of participation, group discussions, artefacts produced during participatory analysis, and field-notes) to describe how participation in a project of self-reflection and self-representation can support dialogical engagement and the demonstration of critical thinking. The thesis explores the relationship between these psycho-social changes and young people's subsequent ability to enact strategies to improve their health and well-being. Findings challenge idealised representations of youth participation, demonstrating that young people's ability to act is mediated and bounded by the health-related contexts in which they live. They also demonstrate a disconnect between youth health priorities and the priorities of the programs 'targeting' them; and point to the importance of HIV-prevention programs working to support 'in-between' spaces where youth and community leaders can connect in order to affect wider social environments. In providing a detailed examination of a Photovoice process, this thesis extends the theoretical basis of an increasingly popular participatory research tool. In analysing the relationship between young people's participation in a research project and their ability to take action on health, this thesis also contributes to social psychological understandings of the pathways through which participation may impact upon health, and in particular affect efforts to prevent HIV.

Investing in Communities Achieves Results fills an important gap in the global knowledge on community level results and resources related to HIV and AIDS. While communities, in spite of their limited resources, have played a key role in the HIV/AIDS response, their contributions and innovative approaches to prevention, treatment, care, and support have not always been the focus of systematic and rigorous evaluations. To address this deficit, a series of studies-including evaluations in Burkina Faso, India, Kenya, Lesotho, Nigeria, Senegal, South Africa, and Zimbabwe-were undertaken over a three-year period (early 2009 to early 2012), helping to build a robust pool of evidence on the effects of community-based activities and programs. A unique feature of this multicountry evaluation was the collaboration between two international organizations (the World Bank and the United Kingdom's Department for International Development) and a major civil society network (the U.K. Consortium on AIDS and International Development). Other attributes that contributed to the successful outcome were the sustained consultation process with civil society and stakeholders at the local, national, and global levels, and the collaboration among high-caliber, multi-disciplinary researcher teams. The book's findings are promising. At varying levels, depending on the country context, the HIV response in communities was shown to improve knowledge and behavior and increase the use of health services- and even decrease HIV incidence. Evidence on social transformation was more mixed, with community groups found to be effective only in some settings. Each study in the evaluation provides a partial view of how communities shape the local response; however, taken together they constitute a significant pool of rigorous evidence on the contributions of communities, community groups, and civil society to the national and global HIV and AIDS response. The studies suggest that communities have produced significant results at the local level, which contribute to outcomes at the national level.

Drawing on a growing consensus about the importance of community representation and participation for ethical research, community engagement has become a central component of scientific research, policy-making, ethical review, and technology design. The diversity of actors involved in large-scale global health research collaborations and the broader ‘background conditions’ of global inequality and injustice that frame the field have led some researchers, funders, and policy-makers to conclude that community engagement is nothing less than a moral imperative in global health research. Rather than taking community engagement as a given, the contributions in this edited volume highlight how processes of community engagement are shaped by particular local histories and social and political dynamics, and by the complex social relations between different actors involved in global public health research. By interrogating the everyday politics and practices of engagement across diverse contexts, the book pushes conversations around engagement and participation beyond their conventional framings. In doing so, it raises radical questions about knowledge, power, expertise, authority, representation, inclusivity, and ethics and to make recommendations for more transformative, inclusive, and meaningful community engagement.
