Introduction : but de l’étude : évaluer s’il est possible d’améliorer le parcours de soins des patients dans un contexte d’urgence palliative à domicile à Nice. Matériel et méthode : il s’agit d'un travail comprenant deux parties. Une étude quantitative, descriptive, transversale, unicentrique, rétrospective pour réaliser un état des lieux de la prise en charge des patients dans un contexte d’urgence palliative à domicile à Nice. Suivie d’un travail quantitatif, descriptif sous forme d’analyse par questionnaire pour faire évaluer par des professionnels de santé la pertinence des propositions d’amélioration de la prise en charge des patients dans un contexte d’urgence palliative à domicile retrouvées dans la littérature. Résultats : nos résultats sont similaires à ceux d’autres études et les patients en situations d’urgence palliative à domicile ont un profil commun. Les patients pour lesquels un appel au service d’urgence est sollicité sont des patients âgés, atteints de maladies chroniques, particulièrement carcinologiques. La pathologie aiguë entrainant l’appel est majoritairement une décompensation respiratoire. La personne appelante est la famille ou un proche dans la majorité des cas et un établissement d'hébergement pour personnes âgées dépendantes (EHPAD) dans un quart des cas. Le questionnement de professionnels de la région aura mis en avant trois leviers d’amélioration : mettre en place un numéro direct accessible 24h/24h aux professionnels pour avis téléphonique rapide par un médecin de soins palliatifs ; développer l'utilisation des prescriptions anticipées personnalisées à domicile ; faciliter la coordination logistique en urgence pour la mise en place des thérapeutiques palliatives à domicile ou en EHPAD, qui n’auraient pas été anticipées. Conclusion : de ce constat et suite à un travail réflexif autour du parcours de soins des patients dans un contexte palliatif à domicile. Il semble intéressant de réunir les acteurs de soins concernés pour discuter des projets en cours sur le territoire et au niveau national. Ensemble avec une approche pluridisciplinaire, on pourrait définir une organisation commune permettant d’améliorer le parcours de soins des patients dans un contexte palliatif à domicile à Nice.
