Le médecin généraliste et le pharmacien officinal sont rarement intégrés à la prise de décisions lors des modifications thérapeutiques des patients à l'hôpital. Une approche innovante pluridisciplinaire ville-hôpital d'optimisation de la prise en charge médicamenteuse du patient âgé a été mise en place au sein de l'hôpital de jour gériatrique du centre hospitalier Saint Philibert de Lomme. Cette étude s'est intéressée à la faisabilité de ce processus qui comportait des étapes de conciliation médicamenteuse d'entrée et de sortie, entre lesquelles s'intercalait une réunion de concertation pluridisciplinaire téléphonique ville-hôpital. Cette réunion rassemblait le gériatre, le pharmacien hospitalier, le médecin généraliste et le pharmacien officinal, et aboutissait à une évaluation collégiale de la prise en charge médicamenteuse du patient. Après 6 mois d'évaluation, ce processus s'avère chronophage et par conséquent complexe à intégrer dans la pratique quotidienne des professionnels de santé impliqués. Cette révision pluridisciplinaire ville-hôpital a toutefois renforcé les échanges entre les différents acteurs hospitaliers et libéraux prenant en charge le patient. Les modifications thérapeutiques qui en ont découlé sont plus pertinentes et devraient être pérennes. Une optimisation du processus permettrait sa transposition en hospitalisation conventionnelle. Il serait intéressant d'évaluer son impact clinique sur la prévention du risque iatrogène chez le patient âgé.

La proportion de personnes âgées dans la population française ne cesse d'augmenter. La prise en charge médicamenteuse pour ces patients nécessite une attention particulière, du fait des nombreuses pathologies retrouvées, s'additionnant au vieillissement physiologique. Les prescriptions médicamenteuses en découlant sont nombreuses, favorisant les interactions ou les effets indésirables médicamenteux. Le risque iatrogène est donc important, qu'il soit lié au patient, au médicament ou à l'organisation des soins. Le pharmacien est un acteur pour la lutte contre la iatrogénie médicamenteuse, en soins primaires comme à l'hôpital. Ce travail présente un bilan des entretiens d'optimisation thérapeutique effectués par le service pharmacie, pour les patients venant en hospitalisation de jour, au sein de l'hôpital gériatrique de Fourvière (Lyon). Soixante-douze patients venus à l'hôpital de jour ont bénéficié d'un avis pharmaceutique lors d'un entretien, visant à analyser l'ensemble de leurs prescriptions, mais également à évaluer leurs connaissances des traitements et leur observance. Les problèmes retrouvés sur les prescriptions concernaient principalement l'utilisation de médicaments potentiellement inappropriés, le ressenti d'effets indésirables et la non-observance des traitements. Les interventions pharmaceutiques proposées aux médecins, encourageaient une substitution ou un arrêt des thérapeutiques incriminées. Les médicaments les plus retrouvés appartenaient aux classes ATC du système nerveux, du système cardiovasculaire et du système digestif. Dans le courrier de sortie, les avis pharmaceutiques étaient ainsi transmis au médecin traitant. Le recueil de l'acceptabilité des interventions pharmaceutiques par les médecins généralistes n'a cependant pas été possible. Ces entretiens font le parallèle avec les bilans partagés de médication, nouvelle mission du pharmacien d'officine, pour lutter contre la iatrogénie, et ainsi améliorer la prise en charge médicamenteuse des patients âgés.

A l'hôpital gériatrique des Charpennes, Hospices Civils de Lyon, au cours de l'hospitalisation de jour, en collaboration avec les médecins, les pharmaciens hospitaliers proposent une optimisation des thérapeutiques médicamenteuses après avoir établi un historique médicamenteux complet. Le pharmacien d'officine est un acteur majeur du parcours de santé du patient et de son suivi thérapeutique. L'objectif principal était d'évaluer l'impact de l'intégration du pharmacien d'officine dans la prise en charge de la personne âgée venant consulter en hôpital de jour gériatrique et bénéficiant d'une optimisation pharmaceutique. Les patients venant consulter à l'hôpital de jour (HDJ) des Charpennes entre janvier et juin 2016 pour un bilan de chute et/ou mémoire et dont le nom de la pharmacie était disponible ont été inclus. Les pharmacies d'officines ont été contactées pour recueillir le traitement délivré les derniers mois. Les informations ont été standardisées et intégrées aux optimisations pharmaceutiques pour transmission au médecin traitant par l'intermédiaire du compte-rendu d'hospitalisation ainsi qu'au pharmacien d'officine. Pour évaluer la démarche, l'évolution des traitements réellement délivrés au patient a été suivie par un deuxième appel à 3 mois. Enfin la satisfaction des pharmacies d'officine a été recueillie. Quarante patients d'âge moyen 81,8 +/- 6,8 ans ont été inclus, 85% présentaient des troubles cognitifs, 60% vivaient avec leurs conjoints à domicile et 25% vivaient seuls à domicile. Au regard des prescriptions des patients, les taux des médicaments habituels prescrits et délivrés à l'officine ont été augmentés significativement à de 3 mois après l'HDJ respectivement de 84% à 93% (p-value

L’optimisation médicamenteuse est un enjeu majeur de la prise en gériatrique. Cette étude non interventionnelle évalue l'impact d'un outil d'optimisation médicamenteuse dans le service de gériatrie du CHU de Caen, incluant 250 patients de plus de 75 ans hospitalisés dans le service et répartis en deux groupes, "soins courants" ou "optimisation médicamenteuse", ces derniers bénéficiant de la réalisation à l'admission d'une fiche d'optimisation médicamenteuse récapitulant les prescriptions inappropriées (comprenant les médicaments potentiellement inappropriés et les médicaments potentiellement appropriés omis) selon les critères STOPP/START.v2. Le critère de jugement principal était la différentielle de prescriptions inappropriées entre l'entrée et la sortie. La prescription de médicaments potentiellement inappropriés et l’omission de médicaments potentiellement appropriés diminuait pendant l’hospitalisation, sans différence significative entre les deux groupes (-0.9±1.2 vs. -1.3±1.4 ; p=0.12 et -0.2±1 vs. -0.3±1.3 ; p=0.22). On notait, dans le groupe « optimisation », une diminution significative des omissions de médicaments potentiellement appropriés non justifiées (61.4% vs. 49.6% ; p=0.048) et une diminution significative des médicaments potentiellement inappropriés chez les patients polymédiqués avec sept médicaments ou plus sur l’ordonnance d’entrée (-1,1±1,2 vs. -1,7±1,5 ; p=0,02). Notre outil n’a pas permis de diminution significative des prescriptions inappropriées ; cependant, le groupe « optimisation » était comparé à un groupe pris en charge par une équipe de gériatres formée à l’optimisation médicamenteuse.

How the insane asylum became a laboratory of democracy is revealed in this provocative look at the treatment of the mentally ill in nineteenth-century France. Political thinkers reasoned that if government was to rest in the hands of individuals, then measures should be taken to understand the deepest reaches of the self, including the state of madness. Marcel Gauchet and Gladys Swain maintain that the asylum originally embodied the revolutionary hope of curing all the insane by saving the glimmer of sanity left in them. Their analysis of why this utopian vision failed ultimately constitutes both a powerful argument for liberalism and a direct challenge to Michel Foucault's indictment of liberal institutions.
