The second half of the 20th century and the beginning of the 21st century witnessed important changes in ecology, climate and human behaviour that favoured the development of urban pests. Most alarmingly, urban planners now face the dramatic expansion of urban sprawl, in which city suburbs are growing into the natural habitats of ticks, rodents and other pests. Also, many city managers now erroneously assume that pest-borne diseases are relics of the past. All these changes make timely a new analysis of the direct and indirect effects of present-day urban pests on health. Such an analysis should lead to the development of strategies to manage them and reduce the risk of exposure. To this end, WHO invited international experts in various fields - pests, pest-related diseases and pest management - to provide evidence on which to base policies. These experts identified the public health risk posed by various pests and appropriate measures to prevent and control them. Surprising though it seems, the world faces almost as great a threat today from arthropod-borne diseases as it did in the heady days of the 1950s when global eradication of such diseases by eliminating their vectors with synthetic insecticides, particularly DDT, seemed a real possibility. Malaria, for example, still causes tremendous morbidity and mortality throughout the world, especially in Africa. Knowledge of the biology of insect and arachnid disease vectors is arguably more important now than it has ever been. Biological research directed at the development of better methods of control becomes even more important in the light of the partial failure of many control schemes that are based on insecticide- although not all is gloom, since basic biological studies have contributed enormously to the outstanding success of international control programmes such as the vast Onchocerciasis Control Programme in West Africa. It is a sine qua non for proper understanding of the epidemiology and successful vector control of any human disease transmitted by an arthropod that all concerned with the problem - medical entomologist, parasitologist, field technician - have a good basic understanding of the arthropod's biology. Knowledge will be needed not only of its direct relationship to any parasite or pathogen that it transmits but also of its structure, its life history and its behaviour - in short, its natural history. To the entomologist all insects have six legs; the layman tends to use the term "insect" to include the eight-legged spiders and mites. All these creatures are correctly classified as arthropods. Many thousands of the hundreds of thousands of recognised species of arthropods are found in the human environment-domestic, occupational and rec reational. Those species which are obligate parasites of man, the human scabies mite and the head and body lice, produce familiar clinical syndromes. They remain important in medical practice and have been the subject of a great deal of recent research. This is beginning to throw much light on the immunological mechanisms which largely determine the reactions of the host. Dr. Alexander has provided a detailed survey of this work.
