La prise en charge du mélanome métastatique a considérablement évolué ces dernières années avec notamment, la mise sur le marché des thérapies ciblées orales (TCO). Avec ces médicaments administrés à domicile, de nouvelles problématiques pharmacothérapeutiques apparaissent telles que la gestion des effets indésirables, l'adhésion au traitement et le risque d'interactions médicamenteuses avec les autres traitements pris par le patient. L'objectif de ce travail est d'évaluer la faisabilité et l'utilité d'une consultation pharmaceutique pour sécuriser la prise en charge des patients sous TCO. Au total, 16 patients ont été inclus : 12 patients à l'initiation de la TCO et 4 patients en cours de traitement. Une consultation pharmaceutique de suivi à un mois a été réalisée pour évaluer l'adhésion et la tolérance du patient au traitement et 3 patients en ont bénéficié. La durée totale de l'activité pharmaceutique a été estimée à 144 min à l'initiation et 87 min lors du suivi. Trente-cinq interventions pharmaceutiques ont été réalisées à l'initiation dont 25 concernaient des interactions médicamenteuses. Cette activité s'est révélée chronophage et le temps passé est très variable d'un patient à l'autre mais elle permet d'informer les patients sur le bon usage de ces médicaments et grâce à la conciliation médicamenteuse, de nombreuses interactions médicamenteuses ont pu être détectées et ont fait l'objet d'interventions pharmaceutiques. Ce travail montre qu'une prise en charge pluridisciplinaire avec l'intervention d'un pharmacien est nécessaire pour optimiser la prise en charge de ces patients. Une optimisation de chacune des étapes de l'activité est envisagée afin de réduire le temps passé par patient en conservant le même niveau de prestation.
