Reports have suggested that a variety of risk factors, including poor oral hygiene, a history of periodontitis, diabetes, smoking, lack of keratinized mucosa, and implant surface topography, are associated with these diseases. Periodontally compromised patients who do not adhere completely to the supportive periodontal therapy have been found to present a higher implant failure rate, and several studies have shown that periodontal pathogens can translocate from periodontally involved teeth to the peri-implant sulci in partially dentate patients, highlighting the importance of periodontal treatment of the residual dentition prior to placement of dental implants. Recently a new concept was proposed as intraindividual variation exists in core microbiota in peri-implantitis and periodontitis. Au cours des dernières années, l'implantologie a connu de nombreux progrès, notamment sur les résultats concernant l'ostéointégration . Alors que les critères de succès ont été largement décrits, l'analyse des paramètres cliniques a permis de révéler l'existence de remodale muqueux et osseux autour des implants. Certains remodelages peuvent s'avérer physiologiques, d'autres peuvent être au contraire pathologiques et représenter de véritables maladies péri-implantaires ou "péri-implantites". Nous proposons dans cette thèse, grâce à une analyse de la littérature, de confronter les maladies péri-implantaires aux maladies parodontales afin d'extraire leurs points communs et principalement leurs différences. Ces maaldies péri-implantaires impliquent la présence de bactéries parodontopathogènes mais aussi dans certains cas, la présence de S. aureus. L'étiologie et le développement de ces maladies restent encore mal connus. L'architecte péri-implantaire engendre des régulations particulières du micro-environnement parodontal. Au niveau osseux, elles se manifestent par la perte progressive de l'os autour de l'implant à la suite d'une réaction inflammatoire. Ces complications biologiques sont liées à un certain nombre de facteurs de risque, comme le tabac ou l'existence d'un antécédent de maladie parodontale. Les traitements, quant à eux, sont empruntés à la parodontologie, or les résultats ne sont pas aussi prévisibles. L'émergence de telles différences nous amènent à conclure qu'il semblerait pertinent de développer une prise en charge spécifique à cette pathologie.
