They are tokens of healing and hope, as well as valuable commodities. La polymédication liée à la polypathologie est le premier facteur de risque de la iatrogénie médicamenteuse, qui est responsable de plus de 10% des hospitalisations du sujet âgé. Le mésusage des médicaments et le manque d'adhésion au traitement peuvent favoriser la survenue d'effets indésirables. Ils sont fréquents et graves chez la personne âgée mais sont en grande partie évitables. Afin d'améliorer l'observance des personnes âgées fragiles prises en charge par le RSG de Lille Hellemmes Lomme, un outil de présentation du traitement médicamenteux a été mis au point en partenariat avec un groupe de médecins généralistes du Nord Pas de Calais. Cet outil « Mon Traitement » récapitule la liste des médicaments ainsi que les données utiles à une prescription optimisée. Il est envoyé avec le courrier du RSG au médecin traitant qui, après validation, le remet au patient ou à son aidant au domicile pour la préparation et l'administration des médicaments. L'étude a pour objectif principal d'analyser la revue d'ordonnance faite par le RSG et secondairement d'évaluer la qualité de remplissage et d'établir un profil sociodémographique des patients pris en charge par le RSG. En deux ans, 244 outils « Mon Traitement » ont été complétés par les gériatres du RSG et adressés aux médecins traitants. Il apparaît que 72% des patients sont polymédiqués, dont 37% de médicaments prescrits inadaptés ; 98% présentent des facteurs de mauvaise observance. La population prise en charge par le RSG est représentative des patients fragiles, à risque d'effets iatrogènes médicamenteux. 57,8% des ordonnances contiennent une prescription de psychotropes (de 10,6% avec plus de 2 psychotropes). Après la revue d'ordonnance, on observe une baisse de prescription de 12% de médicaments inappropriés et de 2% de psychotropes. 63,5% des fiches sont considérées suffisamment remplies avec l'ensemble des données utiles. Cette étude révèle aussi que la démarche est difficile à poursuivre au long cours : le temps de remplissage est important, l'outil sous forme papier ne permet pas de mises à jour régulières et les fiches sont rarement retrouvées au domicile. Un logiciel informatisé intégrant l'outil est en cours de développement pour optimiser le partage d'informations entre les intervenants.
