La mise en oeuvre d'un processus d'analyse stratégique mêle des informations structurées et des données non structurées. Le calcul et la raison impliquent un effort de conception collective, ainsi que l'alternance entre l'explication positive et la compréhension. La logique linéaire d'usage courant en gestion doit faire place à une logique paradoxale qui implique les acteurs externes et internes de l'hôpital. La théorie partielle appliquée à l'hôpital s'inscrit dans une théorie moyenne spécifique au secteur de la santé. Nous nous sommes référés à plusieurs études positives et hypothético-déductives récentes. Les caractéristiques de l'environnement hospitalier universitaire ont été déduites d'interviews effectués au sein de 14 hôpitaux. Une phase quantitative d'analyse des représentations de 1 230 dirigeants professionnels et gestionnaires a été assurée auprès de 290 établissements de grande taille en France et en Belgique. Trois enquêtes, ainsi qu'une étude positive des comportements de consommation des services de soins ont permis de valider les attributs discriminants des services offerts par l'hôpital. La recherche intègre : les mécanismes des pouvoirs de monopole, la gestion des changements dans les entreprises complexes, les caractéristiques des organisations de professionnels, la théorie de l'agence.Le processus de management stratégique, la composition du sommet stratégique et les méthodes d'analyses stratégiques proposées devront probablement subir des aménagements lors de leur mise à l'épreuve. Cependant, différentes hypothèses vérifiées dans le secteur marchand ont pu être validées dans le cas de l'hôpital universitaire. Nous espérons également avoir ouvert de nouvelles voies de recherche en management stratégique hospitalier, adaptées au contexte européen.
