La ventilation mécanique invasive est associée à une morbi-mortalité importante. Plusieurs pistes, dont le lever précoce, existent pour réduire le temps passé par les patients sous ventilation. L'objectif était d'évaluer les modalités pratiques d'un protocole de lever au fauteuil des patients bénéficiant d'une ventilation mécanique invasive en Réanimation Chirurgicale. Etaient inclus les patients de Réanimation Chirurgicale de l'hôpital Central de Nancy, bénéficiant d'une ventilation mécanique invasive, qui étaient levés au fauteuil. Chaque lever s'effectuait selon une procédure écrite précise, disponible dans le service. Les données clinques des patients étaient recueillies avant, pendant, et après la procédure de lever. Au terme de chaque séance, une note de 0 à 10 était donnée par un médecin, un(e) infirmier(ère), un(e) aide-soignant(e) et un(e) kinésithérapeute, afin d'évaluer de manière subjective le bénéfice de la séance pour le patient. Cent patients ont été inclus. Les motifs d'admission en Réanimation étaient majoritairement des pathologies chirurgicales, nécessitant des équipements techniques lourds. 551 séances de lever étaient répertoriées ; le temps moyen pour lever les patients était de 6,8 minutes, nécessitant en moyenne 2,8 membres du personnel. Aucune complication n'avait été observée pour 93,5% des séances. La note moyenne octroyée par le personnel quant à l'évaluation du bénéfice de chaque séance était de 6,1 sur 10. L'admission pour polytraumatisme, pathologie neurologique, et le port d'une minerve étaient les seuls facteurs significativement associés à un allongement du temps pour lever. Aucun facteur n'était prédictif de la survenue d'une complication. Notre étude montre que le lever au fauteuil des patients sous ventilation invasive en Réanimation Chirurgicale est une procédure pouvant être réalisée de manière routinière et sûre pour les patients, en suggérant un bénéfice. La lourdeur de la procédure n'est pas un frein à cela, si tant est que toute l'équipe médicale et paramédicale adhère à un protocole de lever rigoureusement appliqué.
