L'inadéquation entre l'évolution de la couverture contraceptive des femmes et la stabilité du taux de grossesses non désirées fait évoquer une inadéquation entre les modalités contraceptives et les besoins et attentes des femmes. Dès 2004, l'ANAES insistait sur la pratique d'une décision partagée basée sur le modèle BERCER en consultation pour aider les patientes à participer activement au choix de leur contraceptif. La HAS a développé en 2014, sur cette base, un outil d'aide à la décision médicale partagé [sic]. Il comprenait d'une part un formulaire de préparation à la consultation pour le patient, regroupant différentes questions sur ses attentes en matière de contraceptifs et différentes sources d'informations, d'autre part un guide de déroulement de la consultation pour le praticien reprenant les étapes du modèle BERCER. Cet outil n'a jamais été testé en pratique courante. L'objectif principal de cette étude était d'observer la manière dont les professionnels de santé allaient s'approprier l'outil en consultation et mettre en évidence les apports et les difficultés d'utilisation de celui-ci. Les objectifs secoàndaires étaient d'évaluer la satisfaction des patientes et suggérer des améliorations de l'outil à partir des commentaires des professionnels de santé. Une méthode mixte a été utilisée, des praticiens de santé ont utilisé l'outil avec des patientes en demande de contraceptif. Après chaque inclusion de patiente, un entretien semi dirigé était réalisé avec le praticien. Parallèlement, les patientes étaient rappelées pour répondre à un questionnaire. Chaque praticien a été invité en fin d'étude à répondre à un questionnaire quantitatif colligeant les modalités de consultation et leur satisfaction. Résultats : 10 praticiens de santé dont 6 médecins généralistes, 2 gynécologues et 2 sages-femmes ont accepté de participer à l'étude. 28 patientes ont été inclues d'avril à juin 2019. Les praticiens ont principalement évoqué des difficultés à la collecte du volet patient après sa transmission et à l'utilisation du volet médecin jugé trop littéraire, inadapté à une utilisation en consultation. Le volet patient a présenté un apport certain, autant pour les professionnels de santé que pour les patientes, il ciblait les attentes des patientes, guidait la consultation et constituait une base d'échange d'information entre les intervenants. Les patientes ont été satisfaites de la consultation. Des améliorations telles qu'une version dématérialisée, imagée, en plusieurs langues seraient à apporter à l'outil afin de le rendre plus facile d'utilisation. En termes de contenu, l'ajout d'informations sur les différents contraceptifs disponibles permettraient d'accroitre le bénéfice apporté et de faciliter l'échange avec les patientes. Conclusion : l'outil développé par HAS présente un apport pour les praticiens et les patientes. Il facilite la mise en place d'un processus décisionnel partagé dans le cadre de la prescription de contraceptif. Il doit cependant être amélioré afin d'être amélioré afin d'être utilisable plus facilement en pratique courante.
