This report summarizes information received from 106 malaria-endemic countries and from malaria control partners. It highlights continued progress made towards meeting international targets for malaria control to be achieved by 2010 and by 2015. International funds disbursed for malaria control increased from $200 million in 2004 to $1.5 billion in 2009. Since 2008, more than 289 million insecticide-treated mosquito nets have been delivered to sub-Saharan Africa, enough to protect three quarters of the 765 million persons at risk of the disease. Over the last decade, 11 countries in the Africa Region and 32 countries in other regions showed reductions of 50% or greater in either confirmed malaria cases or malaria admissions and deaths. Malaria control is making a major contribution to reducing mortality in children less than 5 years old and to attaining the health-related Millennium Development Goals.

Introduction: Avant 2 ans, 50 à 60% des enfants sont suivis régulièrement par un pédiatre et 50% par un médecin généraliste. Après l'âge de 2 ans, 20% des enfants sont suivis régulièrement par un pédiatre et 80% par un médecin généraliste. La Protection Maternelle et Infantile gère 10 à 15% des enfants de 0 à 6 ans. Des études quantitatives françaises se sont intéressées aux critères de choix des parents pour le professionnel assurant le suivi de leur enfant mais n'ont pas exploré leurs attentes et leur ressenti. L'objectif était de décrire les attentes et le ressenti des parents concernant le suivi de la santé de leur enfant de 0 à 2 ans. Méthode : Étude qualitative par entretiens semi-dirigés de 29 parents rencontrés en réseau de périnatalité et en maternité. Résultats : L'étude a été menée jusqu'à saturation de données. Les principales attentes des parents concernant le professionnel assurant le suivi de leur enfant de 0 à 2 ans étaient une relation de qualité et centrée sur l'enfant, le respect de la déontologie, les compétences du professionnel et sa disponibilité. Les principales attentes concernant le contenu des consultations de suivi étaient la réassurance et l'étayage parental, les conseils sur l'alimentation, le suivi des vaccinations et la prévention, la surveillance de la croissance staturo-pondérale et du développement psychomoteur. Les parents attendaient également un diagnostic exact des pathologies aiguës et une prise en charge rapidement efficace. Le médecin généraliste répondait généralement à ces attentes pour les parents ayant fait le choix d'un suivi de l'enfant par le médecin généraliste. Toutefois, ils avaient parfois le sentiment que ses conseils étaient moins précis et l'examen clinique moins approfondi que ceux du pédiatre. Les parents consultant le pédiatre pour le suivi de leur enfant attribuaient au médecin généraliste uniquement un rôle de soin. L'étude a également montré une méconnaissance des parents concernant la place des professionnels dans la santé de l'enfant et le plan de prévention de la santé de l'enfant. Conclusion : Afin d'optimiser le suivi de la santé de l'enfant, il est donc nécessaire de mieux informer les parents sur la place et les compétences des professionnels, notamment sur le rôle du médecin généraliste dans le suivi de l'enfant, et sur le plan de la prévention. Il est indispensable d'améliorer la formation initiale et continue des médecins généralistes en termes de suivi de l'enfant.
