48 patients porteurs d’une insuffisance respiratoire chronique grave ont été soumis à une ventilation mécanique au long cours par trachéotomie entre octobre 1975 et septembre 1986. Un groupe plus homogène de 35 patients, ventilés à domicile, est plus particulièrement étudié. Il s’agit de 15 femmes et de 20 hommes, âgés de 26 à 82 ans (59 ans ± 11) au moment de leur retour à domicile ; 22 patients ont une insuffisance respiratoire chronique à prédominance restrictive ; et 13 patients ont une insuffisance respiratoire chronique à prédominance obstructive. La trachéotomie a été pratiquée dans tous les cas chez des patients ayant nécessité une ventilation mécanique pour décompensation. Il s’agissait de la première ventilation mécanique dans 68.5% des cas. La durée journalière moyenne de ventilation mécanique à domicile est de 12h, complétée par une oxygénothérapie nocturne et/ou diurne dans 65.7% des cas. La diminution de la durée moyenne de séjour hospitalier par sujet et par an après le retour à domicile est un témoin de l’efficacité de cette méthode. A 5 ans, le taux global de survie (méthode actuarielle) est de 60.4%. Il est de 66.6% pour les insuffisants respiratoires chroniques à prédominance obstructive. Les décès sont dûs généralement à l’évolution de l’insuffisance respiratoire chronique pour les obstructifs et à une autre cause pour les restrictifs.
