La maladie de Horton est la plus fréquente des vascularites. A partir de huit cas de maladie de Horton diagnostiqués entre janvier 2000 et décembre 2003 dans un service de médecine interne, nous avons revu les symptômes de la maladie, les thérapeutiques, les complications et le rôle du médecin généraliste dans la prise en charge de cette pathologie. Les manifestations cliniques sont très diverses : il peut s'agir d'un tableau clinique typique avec des céphalées, une hyperesthésie du cuir chevelu, une claudication de la mâchoire, une amaurose transitoire et un syndrome fébrile, ou d'un tableau clinique tout à fait spécifique avec une altération de l'état général, de la fièvre et un amaigrissement. Cette diversité de symptômes en fait une maladie difficile à diagnostiquer et met le médecin généraliste en difficulté. La gravité de cette maladie est lié au pronostic visuel du patient qui est mis en jeu. L'examen complémentaire qui permet d'apporter la certitude diagnostique reste la biopsie d'artère temporale. Le traitement de référence de la maladie de Horton est la corticothérapie pour une durée de 18 à 24 mois. De nombreuses complications peuvent survenir lors du traitement et de sa décroissance. Principalement des complications infectieuses et rhumatologiques. L'évolution de la maladie est habituellement favorable sous corticothérapie. Le médecin généraliste joue un rôle essentiel dans la prise en charge de cette pathologie : il doit évoquer le diagnostic, adresser le patient en milieu hospitalier pour confirmer le diagnostic, prendre en charge le suivi de la maladie avec les éventuelles complications et s'assurer du bon déroulement de la décroissance de la corticothérapie

The prevalence of autoimmune diseases and rheumatic conditions is constantly increasing. Autoimmune diseases affect approximately 7-10% of the population of the United States, while more than 50,000,000 American adults suffer from some type of arthritis.
