There are many textbooks on fractures in adults, but few deal with those in children and young persons. Publications on fractures in children are usually written by orthope dic surgeons who are familiar with the problems presented by the growing skeleton. The principles taught in the best known works on fractures in children by BLOUNT (1954), RETTIG (1957), CHIGOT and ESTEVE (1957), EHALT (1960), POLLEN (1973), and RANG (1974), differ from each other only slightly. The controversy which has sur rounded the treatment of fractures in adults in recent years has never extended to chi1drens' fractures. Skillful nonoperative treatment of the latter leads to uneventful healing in the majority of cases, which cannot always be said of similar injuries in adults. "Considerable skill" is required to induce nonunion in a child, and its occur rence always results from a serious error of management. In the adult, prolonged immobilization frequently leads to muscle atrophy, joint stiffness, and disturbances of the peripheral innervation and circulation. Fracture treatment by internal fixation in the manner originally described by LANE (1894), LAMBOTTE (1892, 1907, 1913), KONIG (1905, 1931), and DANIS (1932, 1949) has gained wide acceptance only in the last 15 years. Since 1958, MULLER, ALLGOWER, WIL LENEGGER, and the other members of the Swiss Association for the Study of Internal Fixation (ASIF) have been developing techniques and instruments for the operative treatment of fractures.

Although the number of published papers on traumatic hip disloca tion in childhood is large, our actual current knowledge on this inju ry in this age group is still full of obscure points and unsolved prob lems. Luxation of the hip of traumatic origin in children is infrequent, and the small number of cases reported by authors and institutions, as well as the usually short follow-up in many cases, are responsible for the many doubts and questions, especially related to complications and outcome following the injury. Consequently the treatment, especially the postreduction management, has not been adequately defined. Dr. Antonio Barquet has made the great effort to analyze the ex perience gained from 38 cases in Uruguay and to review more than 500 references related to the subject, covering more than 1400 cases. He began to study this topic in 1976. It was the subject of his postgraduate monograph in 1979, which was followed by a statisti cal analysis of the major complication, i. e., avascular necrosis of the proximal femoral extremity, which was presented as his doctoral thesis in 1981. In addition, he has published numerous papers deal ing with several aspects of traumatic hip dislocation in children.

The first part of this manual deals with the experimental and scientific basis and the principles of the AOjASIF method of stable internal fixation. It deals with the function and main use of the different AO implants, the use of the different AO instruments, and with the essentials of the operative technique and of postoperative care. It also discusses the handling of the most important postoperative complications. The second part deals at length with the AO recommendations for the operative treatment of the most common closed fractures in the adult. This has been organized in anatomical sequence. In the last 10 years, total prosthetic replacement has become the hip oper ation for many but not all surgeons. In our clinic the increasing frequency of TP has been paralleled by a reduction in the frequency of 10 and HA. For the experienced orthopaedic surgeon and hip surgeon, hip disease has so many facets that it could not possibly be tackled with one operation alone. Thus, HA seems recently to have won renewed recognition. It has once more become the procedure of choice in specific cases, possibly because of certain disadvantages and complications associated with TP which have somewhat tempered the original optimism. Currently, the risk of infection or loosening of the pros thesis is such that the operation cannot justifiably be recommended for patients under 55-60 years of age. In contrast with HA, in the majority of cases the results of TP or 10 are not fully predictable. The loss of joint motion is offset by two important advantages: -complete freedom from pain - full weight bearing is possible on the arthrodesed hip. On following up our HA operations from 1961 to 1971, we examined the development of the operative procedures in our clinic. We found that the range of indications had narrowed as stricter criteria were applied in choosing patients for operation. Adequate and precise operative tech niques were developed. We studied the biomechanics of the hip joint exten sively (effect of arthrodesis on the statics and dynamics of the hip joint and neighbouring joints).
