La thèse présente les contextes de mise en place de la Réduction des Risques en France, Espagne et Angleterre et documente les impacts de cette politique sur le nombre d’ overdoses d’héroïne et la prévalence du VIH et du VHC chez les usagers de drogues par voie intraveineuse (UDIV). L’introduction des traitements de substitution a été suivie d’une réduction du nombre de décès par overdose d’héroïne en France et en Espagne. L’impact a été différé en Angleterre. Un cadre trop lâche de la prescription de méthadone dans ce pays et une supervision insuffisante, au moins jusqu’en 2000, ont nuit à l’efficacité des traitements. La prévalence du VHC reste très élevée chez les UDIV. En revanche, les traitements de substitution et une meilleure accessibilité au matériel d’injection stérile semblent aussi avoir eu un impact déterminant dans lutte contre la propagation du VIH, particulièrement en Angleterre où la distribution de seringues a été massive et gratuite dès le début de l’épidémie.
