Introduction : Les consultations urgentes sont fréquentes provoquant une saturation des services de secours et d'accueil des urgences. Ces consultations de premiers recours impactent également les médecins généralistes. Dans certaines situations, le médecin généraliste a recours au Service d'Aide Médicale Urgente (SAMU) pour être assisté par un Service Mobile d'Urgence et de Réanimation (SMUR) auprès de son patient. L'objectif principal de l'étude était d'identifier les situations et les facteurs incitant un médecin généraliste à recourir au SMUR. Méthode : Nous avons réalisé une étude rétrospective et observationnelle sur le département des Bouches-du-Rhône entre le 1er mai et le 31 décembre 2017. Étaient inclus les médecins généralistes ayant demandé l'intervention d'un SMUR dans leur cabinet. À l'aide de la relève du SAMU, nous avons pu identifier ces médecins généralistes auxquels nous avons envoyé un questionnaire. Résultats : Trois cent soixante-seize interventions du SMUR ont été incluses et 166 médecins généralistes ont complété le questionnaire. Les motifs d'appel étaient par ordre de fréquence : les douleurs thoraciques, les détresses respiratoires et les troubles du rythme. Le taux de médicalisation de l'ensemble de notre étude était de 54 % contre 40 % tous lieux d'interventions confondus. Les médecins avaient appelé le SAMU dans le souhait d'un transport médicalisé dans 97 % des cas. Ils souhaitaient également l'apport de matériel et/ou la réalisation de gestes. Différents sous-groupes ont été créés. Les médecins formés à l'urgence avaient un taux de médicalisation de leurs patients significativement plus élevé. Le nombre d'appels au SAMU-Centre 15 augmente, le régulateur doit décider en tenant compte de la balance bénéfice-risque entre l'intérêt individuel et collectif par son interrogatoire médical d'où l'intérêt de rechercher des aides à la décision notamment par des techniques d'analyse phonatoire. L'objectif de cette étude a été de créer un modèle prédictif d'envoi de service mobile d'urgence et de réanimation par analyse vocale de l'appelant au SAMU en utilisant une régression logistique binaire. Cette étude rétrospective monocentrique non interventionnelle réalisée au SAMU 35 du CHU de Rennes de janvier à mai 2019 a recueilli 1000 enregistrements dont 724 enregistrements analysables (458 avec envoi d'un SMUR et 266 sans SMUR). Un modèle prédictif d'envoi de SMUR a été créé par régression logistique binaire. Les résultats obtenus montrent que certains paramètres vocaux sont pertinents avec un modèle prédictif final retrouvant 62,9% d'évaluations concordantes avec les données observées.
