La réglementation du secteur des dispositifs médicaux est un domaine complexe et en constante évolution. Ces dernières années, le renforcement de la nécessité d'évaluer le rapport bénéfice-risque des dispositifs médicaux, que ce soit pour l'obtention du marquage CE ou pour la prise en charge par l'Assurance Maladie, a entrainé une augmentation sensible dans les CHU du nombre de recherches cliniques portant sur un dispositif médical. Le pharmacien hospitalier doit avoir une bonne connaissance de la réglementation et comprendre les spécificités de l'évaluation des dispositifs médicaux afin d'appliquer les bonnes pratiques cliniques s'y rapportant. La gestion de ces essais cliniques à la pharmacie doit permettre de faciliter leur réalisation et de maitriser le circuit des dispositifs médicaux faisant l'objet de la recherche. Pour cela, nous avons mis en place à la pharmacie des dispositifs médicaux stériles du CHU de Bordeaux un système de management de la qualité pour assurer une gestion performante des dispositifs médicaux en investigation clinique. Une approche processus nous a permis de décomposer cette activité, de la formaliser et d'enclencher une démarche d'amélioration continue de la qualité. De nouvelles procédures ainsi que des indicateurs de suivi ont été élaborés. Parallèlement, nous avons conçu un logiciel de gestion des dispositifs médicaux en investigation clinique qui a pour but d'obtenir une traçabilité des étapes du circuit de ces dispositifs médicaux. Ce logiciel sera également une aide pour leur comptabilité, leur suivi au cours de l'essai clinique ainsi que pour la mesure de l'activité. Une collaboration étroite avec les investigateurs et les promoteurs des essais cliniques est nécessaire et attendu grâce à cette nouvelle organisation, afin que le service pharmaceutique rendu soit le plus satisfaisant possible.

La sécurisation du circuit de traçabilité des Dispositif Médicaux Implantables (DMI) dans les établissements de santé est un enjeu primordial compte tenu de la nécessité de traçabilité sanitaire, financière et de bon usage. Afin de répondre au mieux à la réglementation et pour améliorer la maîtrise de son circuit des DMI le CHU de Bordeaux s'est engagé dans l'acquisition d'un outil pharmaceutique de gestion des stocks et dépôts de DMI, associée à une réception centralisée des DMI à la pharmacie. Après la présentation de l'état des lieux du circuit actuel des DMI au CHU de Bordeaux, nous présenterons l'avancée du projet qui s'est articulé autour de trois volets : ressources humaines, logistique et informatique. Une auto-évaluation de maîtrise de risque de notre circuit des DMI a été réalisée pour quantifier l'impact de l'acquisition du logiciel dans notre structure. L'acquisition prochaine de ce logiciel va nous permettre de répondre aux exigences réglementaires, de sécuriser totalement notre circuit de traçabilité et d'envisager l'arrivée de la codification UDI prévue pour 2022.
