Background. The loss of spouse is frequent in aging people and is associated with a higher risk of mortality. This association has been well described in many populations but the studies that exposed French patients are rare. Moreover, to our knowledge, none has focused on rural and semi-rural patients which account for an important part of some French departments like Ardennes. In this study, we aimed to show the relation between widowhood and mortality in rural and semi-rural patients from the Ardennes French department. Patients and methods. Nine GP’s included 104 widowers paired with 103 non exposed patients and prospectively followed them during one year. Sociodemographic and medical characteristics were collected at baseline. During follow-up, morbidity occurrence and mortality were reported. A statistical analysis was performed to compare widowers with controls. Results. The two groups were similar at baseline regarding medical and sociodemographic parameters except for occupation with more artisans in widowers and more employees in controls. There were more deaths in the widowers group (p=0.02). In our cohort study, spousal bereavement appeared as an independent risk factor of death (p=0.02) after adjustments for onset of cardiopathy, vascular disease and respiratory insufficiency. Cardiovascular diseases were the main risk factor of death in both groups and depression as assessed by antidepressive therapy prescriptions was an important factor of the excess risk of death after spousal loss. Discussion. We reported widowhood is a risk factor of death in rural and semi-rural French patients. Widowers’ mortality is associated with cardiovascular diseases as opposed to control group. The depression also seems to be a factor of worsening health in the widowers group. These results suggest that a specific caregiving of widowers could improve their health status and reduce mortality associated with spousal bereavement.
