Les personnes âgées dépendantes ont souvent dans la bouche des dépôts microbiens épais, qui sont difficiles à nettoyer pour les aides-soignants des Établissements d'Hébergement pour Personnes Âgées Dépendantes (EHPAD). La formation et la reconstitution rapide de ces dépôts sont favorisées par des conditions caractéristiques du grand âge et de la dépendance, qui empêchent le nettoyage physiologique de la bouche (bouche sèche, dents mobiles ou cariées, prothèses non fonctionnelles, médicaments, aliments mous et mixés). C'est pourquoi, malgré tout le soin apporté, 72 % des soignants trouvent que l'hygiène bucco-dentaire des résidents est insuffisante. Cette étude observationnelle dans le cadre des soins courants avait pour objectif de valider une fiche simplifiée, pour que les aides-soignants en charge des soins d'hygiène bucco-dentaire puissent comparer un score d'état bucco-dentaire au score de l'état oral précédent. Le critère d'évaluation principal était l'observation qualitative, sur graphe, de l'évolution du score pendant 13 semaines consécutives (9 fiches remplies par les aides-soignants puis 4 par l'étudiant). Parmi les 57 résidents en long séjour éligibles à l'étude, 33 ont été analysés (au moins 4 fiches remplies). Sur un score moyen, le graphe a montré que les mauvais scores étaient améliorés la semaine suivante. Ce travail confirme l'intérêt de valoriser par un score chiffré le travail des aides-soignants, ce qui augmente leur motivation pour effectuer les soins de bouche. En améliorant la lutte contre les réservoirs microbiens oraux, ce nouvel outil devrait aider à lutter contre la perte d'appétit, la dénutrition et les pneumonies d'inhalation chez les résidents en EHPAD.
