Résumé : Contexte : Les douleurs thoraciques sont un motif fréquent de consultation aux urgences pédiatriques, avec des étiologies nombreuses, rarement cardiaques, et des examens complémentaires souvent peu contributifs. Les objectifs de cette étude étaient d'évaluer aux urgences pédiatriques la fréquence de réalisation des examens complémentaires, les différentes causes retrouvées, la place de l'avis cardiologique et l'impact de l'utilisation d'un arbre décisionnel en terme de modifications du nombre d'examens réalisés. Méthodes : L'étude était épidémiologique, rétrospective, observationnelle, monocentrique aux urgences pédiatriques du CHRU de Lille entre le 01 janvier 2013 et le 31 décembre 2014, et incluait les enfants âgés de 3 ans à 15 ans 3 mois présentant dans l'anamnèse de leur dossier la notion de douleur thoracique. Etaient exclus les patients aux antécédents de drépanocytose. L'arbre décisionnel choisi était celui du Royal Children Hospital de Melbourne. Résultats : 440 patients étaient inclus, dont 336 pour lesquels la douleur thoracique était le motif de consultation principal. Un ECG était réalisé dans 74% des cas, pathologique dans 8% des cas réalisés. Une radiographie thoracique était réalisée dans 64%, pathologique dans 22%. Un dosage de troponine était réalisé dans 23%, pathologique dans 3%. Un avis cardiologique était demandé pour 20% des patients, chez lesquels étaient réalisées une échographie dans 16% et une hospitalisation en cardiopédiatrie dans 26%. Les causes de douleur étaient fonctionnelles/pariétales/musculaires/intercostales (51%), pulmonaires (16%), cardiaques (9%), virales (6%), digestives (5%), psychologiques (5%), traumatiques (4%), vagales (4%). L'application de l'arbre décisionnel aurait entrainé une hausse de 14% du nombre d'ECG, de 39% de radiographies thoraciques, de 92% de dosages de troponine. Cette augmentation était moindre chez les enfants dont la douleur thoracique constituait le motif principal de consultation (augmentation de 4% d'ECG, 33% de radiographie thoraciques, et 76% de dosages de troponine). Conclusion : Cette étude permet de mesurer la fréquence de recours des examens complémentaires qui serait plutôt majorée par le recours à un arbre décisionnel, sans qu'on puisse mesurer l'apport diagnostique de ces examens ni celui de l'utilisation d'un l'arbre décisionnel.
