Environ 30 % des patients se présentent aux Urgences avec un courrier d'admission rédigé par un médecin généraliste. De nombreuses études ont été réalisées sur la qualité des courriers médicaux, mais quels sont vraiment les demandes des médecins généralistes ? Etude quantitative descriptive et prospective sur 274 courriers d'admission aux Urgences rédigés par des médecins généralistes. Les demandes des médecins généralistes sont formulées précisément sur 74% des courriers et sont souvent multiples. La précision des demandes ne varie pas en fonction de la période de la semaine ou de la qualité du médecin adressant mais elle augmente fortement avec le niveau d'Urgence. Les médecins généralistes sont très précis dans leurs demandes lorsque l'Urgence est avérée. Pour des problèmes non-urgents, les demandes sont plus vagues et la nécessité d'un passage par les Urgences peut être remise en question. Les médecins adressant ont dans ce cas recours aux Urgences pour accélérer la prise en charge du patient en restant dans le domaine.

Objectif:etudier le contenu des courriers d'admission des patients adressés et reçus à une structure d'Urgences.Méthode: 304 courriers de patients adressés et reçus par la structure d'Urgences du Centre Hospitalier de Basse-Terre, Guadeloupe , ont été recueillis sur une période de deux mois.L'analyse de ces courriers est réalisé selon les critères définis par la Haute Autorité de Santé en matière de courriers médicaux à laquelle s'ajoute une analyse selon le type de médecin rédacteur, selon la discipline concernée (médicale, chirurgicale ou orthopédique) et selon la classe d'âge des patients. Résultats : la majorité des courriers font un recto, sont manuscrits et lisibles.Les critères les plu_s renseignés sont l'identité du rédacteur (99%), la date de rédaction (99,7%), l'identité du patient (99%) et le motif de demande de prise en charge (98%). Les données médicales sont les moins renseignées : l'anamèse (57%), les antécédents (47%), l'examen clinique (39%), le traitement au long court (32%) et le traitement des symptômes aigus (14%).Les courriers les moins détaillés ont été rédigés par les médecins traitants. Les courriers les plus informatif concernent une prise en charge médicale, et les patients agés de plus de 75 ans. Conclusion : Dans cette étude, les recommandations concernant les courriers d'admission sont diversement appliquées. Ces résultats pourraient être améliorés par la généralisation d'une lettre type dont la rédaction pourrait être facilitée par l'utilisation du dossier médical informatisé.

Cette étude a pour but d’évaluer la communication ville-hôpital au travers de l’analyse de la lettre d’admission aux Urgences. Un état des lieux a été réalisé à l’aide d’une enquête prospective, se déroulant de janvier à mars 2005, dans l’unité de proximité des urgences du Centre Hospitalier Ariège Couserans, analysant 124 lettres d’admission aux urgences provenant de 30 médecins généralistes différents. Ces courriers sont insuffisamment renseignés pour une prise en charge optimale des patients. Un quart de ces lettres sont difficilement lisibles. Leur contenu informatif varie en fonction du motif d’admission (plus d’informations pour les motifs d’ordres médicaux), et de l’âge du patient (plus d’informations pour les patients de plus de 65 ans). Il n’existe pas de différence entre les lettres provenant des médecins titulaires et des médecins remplaçants. Ensuite un questionnaire a été adressé aux médecins urgentistes de Midi-Pyrénées afin de recueillir leurs attentes au sujet de la transmission de l’information médicale par les médecins généralistes. Les 87 réponses analysées sont consensuelles : l’ensemble des informations de la classique observation médicale est important pour une prise en charge optimale du patient et une lettre-type sur un support courrier papier semble la plus adaptée pour fournir ces informations. Une lettre-type contenant les informations souhaitées est donc proposée en respectant les contraintes légales et déontologiques auxquelles sont soumises les données médicales.
