Les principaux acteurs de la santé de la femme sont les médecins généralistes, les sages-femmes et les gynécologues. Les généralistes ont un savoir théorique de bon niveau en matière de gynécologie à la fois médicale et obstétricale mais peu le mettent en pratique au cabinet. Le territoire picard a en plus une bonne base de formation initiale en matière de gynécologie. Aucune thèse à l'heure actuelle n'a été faite pour évaluer la pratique des gestes techniques gynécologiques les plus utilisés sur ce territoire. Cette étude a pour objectif d'établir un état des lieux des pratiques des gestes techniques de gynécologie en médecine générale sur le territoire picard et de mettre en évidence les éventuelles raisons de non pratique. Une enquête quantitative, descriptive et transversale a été réalisée par un questionnaire envoyé à 400 médecins généralistes sélectionnés. Le questionnaire a été envoyé par voie postale, avec une relance téléphonique par la suite. Cent cinquante-quatre médecins ont participé à l'enquête, dont la moitié présentait un intérêt pour les gestes techniques gynécologiques, le tiers était formé en gynécologie, plus des deux-tiers travaillait en collaboration avec un gynécologue ou avait un spécialiste proche de son cabinet. Nous constatons que plus de la moitié des praticiens posent des spéculums et font des frottis. Le tiers pratique des prélèvements vaginaux, des poses et des retraits de DIU, des poses et des retraits d'implant et le doppler fœtal. La colposcopie, l'échographie, le prélèvement endo-utérin et la biopsie de col seraient des gestes très peu, voire, non pratiqués dans la pratique courante de la médecine générale. Le milieu d'installation semble être l'un des principaux facteurs influençant la pratique des gestes techniques. De manière moins marquée, l'intérêt pour la gynécologie et la formation préalable apparaîtrait comme favorisant cette pratique. Nous remarquons que peu de gestes sont réellement pratiqués en médecine générale. La formation après l'installation semble négligée. On pourrait se demander comment former au mieux les futurs médecins pendant ou à la sortie de l'internat pour qu'ils puissent soulager les gynécologues dans leur pratique quotidienne.
