INTRODUCTION : La définition mondiale des soins primaires a été créée en 1978 à Alma-Ata. On y retrouve les notions de prévention, dépistage, éducation à la santé, prise en charge globale d’une population, et un esprit de justice sociale. La définition a évolué avec le temps et les sociétés, mais semble également différente aujourd’hui selon les modalités d’exercice. L’objectif était de réaliser un état des lieux des perceptions des soins primaires par les professionnels de santé METHODE : Etude qualitative descriptive par focus groups pluridisciplinaires. Le guide d’entretien permettait d’explorer la vision des soins primaires des participants autour de quatre grands thèmes : le premier contact, la continuité des soins, la prise en charge globale et la coordination. Les entretiens ont été intégralement retranscrits et ont servi de corpus pour l’analyse. RESULTATS : Quatre entretiens ont été réalisés avec 16 participants de professions, de mode d’exercice et de degré d’implication universitaire différents. Les soins primaires étaient présentés par les professionnels interrogés comme étant une notion aux limites imprécises. La perception des participants se modifiait selon la situation exceptée pour ce que nous avons appelé les « piliers des soins primaires » : la triade prévention-dépistage-éducation à la santé ; le secteur ambulatoire ; la médecine générale. Le patient est présenté comme un acteur central des soins primaires.

INTRODUCTION: Depuis sa création en 1978, la définition des soins primaires a suivi les évolutions de la société pour s’adapter à de nouveaux besoins. La perception des soins primaires par les professionnels de santé semble différer selon leur modalité d’exercice. Notre étude propose un état des lieux de la définition et de la perception des soins primaires par les professionnels de santé français. METHODE: Etude descriptive transversale par questionnaire diffusée de septembre à décembre 2020. 585 participants inclus. Le recueil de la perception des soins primaires a été effectué par l’intermédiaire de mots clés, de QCMs, QCU et mise en situation par vignettes cliniques. L’échantillon était caractérisé par l’âge, le sexe, la profession, les modalités d’exercice et l’implication universitaire. RESULTATS: Les fonctions de soins les plus associées aux soins primaires étaient « la prévention », « le dépistage », « l’éducation à la santé », « l’orientation dans le système de soins ». La médecine générale ressortait comme acteur privilégié des soins primaires. La profession, l’âge et l’implication universitaire des répondants modifiait significativement leur perception des soins primaires. CONCLUSION: La définition des soins primaires est organisée autour de repères fondamentaux, fortement rattachés aux soins primaires et pouvant orienter la représentation d’une situation clinique comme faisant partie des soins primaires. Autour des piliers des soins primaires gravitent des concepts et notions plus subjectives, soumises à interprétation, en partie dépendante des caractéristiques des professionnels de santé.

Introduction : Le vieillissement de la population et la polymédication engendrent des prescriptions potentiellement inappropriées. Celles-ci sont responsables d'effets indésirables graves et de conséquences économiques pour les patients et la société. La déprescription doit être envisagée lorsque des prescriptions inappropriées sont repérées. Objectif : Réaliser un état des lieux de la déprescription en soins primaires en France et mise en perspective avec les principes de la déprescription définis par Coe et al. Méthodes : Nous avons effectué une revue ciblée de littérature grise avec analyse thématique des ressources retenues. La période d'inclusion s'étendait du 1er janvier 2015 au 4 juin 2022. Ont été incluses des ressources de toute nature, françaises, intéressant la déprescription en soins primaires. Nous avons utilisé les sites des sociétés savantes et leur congrès, des sites gouvernementaux, de dépôt de travaux universitaires et de groupes qualités. Résultat : Au total 63 ressources ont été incluses, dont 48 thèses d'exercice. Les thématiques abordées étaient nombreuses. Elles présentaient des freins et des leviers concernant la déprescription, des formations, outils ou supports à disposition des professionnels de santé ou des patients. Certaines classes médicamenteuses spécifiques étaient ciblées. Malgré la complexité du processus de déprescription, de nombreuses voies d'amélioration étaient proposées par les auteurs. La relation médecin malade et la collaboration interprofessionnelle dans le processus de déprescription étaient souvent citées. Parmi les principes de Coe, celui le plus représenté dans nos ressources étaient le principe 0 (Engagement des professionnels et Identification adéquate des patients), le moins représenté étaient le principe 6 (Apport d'un retour aux professionnels). La plupart des ressources faisaient référence à plusieurs principes simultanément traduisant bien l'aspect multidimensionnel de la déprescription. Ces résultats étaient cohérents avec ceux de Coe et al. Conclusion : En France, le concept de déprescription s'intègre peu à peu dans la pratique. Une meilleure connaissance et diffusion du concept pourrait servir de référentiel aux acteurs du système de soins primaires et améliorer la réussite des processus de déprescription.

Mental health and psychosocial support (MHPSS) is a term used to describe a wide range of actions that address social, psychological and psychiatric problems that are either pre-existing or emergency-induced. Our brain is not organized to automatically perceive that each family has an emotional system, fine-tuned by evolution and "valuing" its survival as a whole, as much as the survival of any individual. It is easier to see this emotional system function in ants or mice but not in humans. The emotional system is organized to snooker us humans: encouraging us to take sides, run away from others, to pressure others, to get sick, to blame others, and to have great difficulty in seeing our part in problems. It is hard to see that we become anxious, stressed out and even that we are difficult to deal with. But "thinking systems" can open the doors of perception, allowing us to experience the world in a different way. This book offers both coaching ideas and stories from leaders as to strategies to break out from social control by de-triangling, using paradoxes, reversals and other types of interruptions of highly linked emotional processes. Time is needed to think clearly about the automatic nature of the two against one triangle. Time and experience is required as we learn strategies to put two people together and get self outside the control of the system. In addition, it takes time to clarify and define one's principles, to know what "I" will or will not do and to be able to take a stand with others with whom we are very involved. The good news is that systems' thinking is possible for anyone. It is always possible for an individual to understand feelings and to integrate them with their more rational brains. In so doing, an individual increases his or her ability to communicate despite misunderstandings or even rejection from important others. The effort involved in creating your Mindful Compass enables us to perceive the relationship system without experiencing it's threats. The four points on the Mindful Compass are: 1) Action for Self, 2) Resistance to Forward Progress, 3) Knowledge of Social Systems and the 4) The Ability to Stand Alone. Each gives us a view of the process one enters when making an effort to define a self and build an emotional backbone. It is not easy to find our way through the social jungle. The ability to know emotional systems well enough to take a position for self and to become more differentiated is part of the natural way humans cope with pressure. Now people can use available knowledge to build an emotional backbone, by thoughtfully altering their part in the relationship system. No one knows how far one can go by making an effort to be more of a self-defined individual in relationships to others. Through increasing emotional maturity, we can find greater individual freedom at the same time that we increase our ability to cooperate and to be close to others.
