Les données épidémiologiques actuelles nous indiquent une augmentation constante des maladies neurodégénératives. Ces pathologies comprennent un large spectre de troubles, y compris un déficit de la mémoire sémantique. Les professionnels de santé ont donc besoin d' outils pertinents pour objectiver et comprendre les troubles auxquels ils sont confrontés. Bien que plusieurs outils existent déjà concernant l'évaluation de la mémoire sémantique, il manquait à la liste une batterie permettant d'évaluer cette mémoire des savoirs dans toutes les modalités sensorielles qui l' alimentent. C'est pourquoi l'ECCS a été créé dans le cadre de mémoires d'orthophonie (Bourgey, 2020, Amiot, 2021) par une équipe de chercheure de Mons, Belgique (Basaglia-Pappas et al., 2021). La présente étude avait donc pour but de poursuivre la normalisation de cet outil auprès de participants témoins âgés de 50 ans ou plus, et sa validation auprès de patients présentant une aphasie dégénérative, notamment une maladie d'Alzheimer (MA) au stade léger (MMSE > 20) et une aphasie primaire progressive variant sémantique (APPvs), afin de permettre au clinicien d'élaborer une prise en soin dans le but d'aider au mieux le patient et son entourage. Les statistiques réalisées montrent que les patients interrogés obtiennent comme attendu des scores inférieurs aux participants témoins, au score global de l'ECCS mais également dans toutes les modalités dans lesquelles sont proposées les épreuves (visuelle imagée, écrite, et autres modalités sensorielles), et à chaque épreuve. Ces résultats, en objectivant un déficit quelle que soit la modalité de présentation, viennent confirmer une atteinte du système sémantique dans la MA et l'APPvs et la sensibilité de l'ECCS pour détecter, même à un stade léger, des difficultés lexico-sémantiques. Ainsi, l'ECCS est un outil pertinent pour participer au dépistage des troubles de la mémoire sémantique dans la MA et l'APPvs.

Ce travail porte sur l'élaboration et l'exploitation d'une base de données de familiarité. Un questionnaire a été créé et diffusé à une population témoin. 114 concepts, majoritairement issus d'épreuves langagières, ont été soigneusement contrôlés pour répondre à des critères psycholinguistiques (fréquence, concrétude et catégorie grammaticale). La participation de 294 sujets de NSC3, regroupés selon le sexe et deux tranches d'âge, a permis de recueillir des données de familiarité pour cet échantillon de la population. La consigne et l'échelle permettant la cotation des concepts sont inspirées de la méthodologie de Rossion et Pourtois (2004). Une fois la base de données créée, nous avons étudié les influences de variables sociodémographiques et psycholinguistiques sur la familiarité. Une exploitation clinique des données recueillies a ensuite été entreprise. Pour cela, nous avons étudié les résultats à l'épreuve de jugement de synonymie de quatre patients atteints de pathologies neurodégénératives (maladie d'Alzheimer, aphasie primaire progressive à variante sémantique et aphasie primaire progressive logopénique). L'analyse des résultats révèle une influence particulièrement robuste des variables psycholinguistiques sur la familiarité. Un effet des variables sociodémographiques est également objectivé mais dans une moindre mesure. Les résultats de l'étude clinique ne corroborent pas les conclusions des recherches antérieures. Un effet de familiarité est retrouvé uniquement chez l'un des patients atteints de la maladie d'Alzheimer. Des cohortes plus importantes de patients permettraient éventuellement de dégager une tendance plus marquée quant à l'influence de la familiarité sur les performances langagières.

Le DTL-V est un test de première ligne destiné à dépister les troubles du langage chez les personnes âgées. Il s’agit du premier test permettant un screening complet du langage. Réalisé en amont d’une évaluation cognitive, il permet de prendre en compte une éventuelle atteinte langagière. La présente étude a contribué à sélectionner les items à l’issue d’une phase de pré-test. Nous avons analysé les effets des variables socio-démographiques (sexe, Niveau Socio-Culturel (NSC) et âge) chez les sujets contrôles (SC), établi les premiers scores seuils des tâches de compréhension et de langage écrit et étudié des cas de patients. La population étudiée comprend 260 SC, 3 patients ayant une aphasie primaire progressive et 2 patients Alzheimer. D’après la littérature, nous nous attendions à un effet positif du NSC, un effet négatif de l’âge et une absence d’effet de sexe chez les SC. Les résultats de la normalisation mettent en évidence un effet du NSC sur les tâches de compréhension et de langage écrit, alors que les effets du sexe et de l’âge ne sont que très peu présents. Les normes ont été établies en prenant en compte les différences de performances liées aux variables socio-démographiques. Nous avons confronté les performances des patients à ces normes et aux hypothèses établies selon les profils typiques de leur pathologie. De plus, ces résultats seront intégrés à une normalisation et une validation multicentriques francophones.
