L’information à la santé est un souhait des patients et un droit. Elle est pour le médecin un devoir, une obligation légale et une étape incontournable à toute démarche préventive. L’information écrite semble être un complément indispensable à l’information orale. Elle améliore la qualité des soins. Cependant, il n’existe pas de documents-patients destinés à la médecine générale, valides scientifiquement, compréhensibles et efficaces en terme de communication. Nous avons donc élaboré une méthodologie et un cahier des charges de fiches idéales puis réalisé 125 fiches traitant des motifs de consultation les plus fréquents en médecine générale. Chacune a été relue par plusieurs médecins et a répondu à un score de lisibilité favorable. Ces fiches seront remises par le médecin généraliste lors de la consultation afin d’améliorer la compréhension du patient, son adhésion au traitement; sa satisfaction; de rassurer les jeunes médecins sur l’information donnée dans un climat médicolégal croissant.

Introduction : l'information écrite améliore les comportements en santé des patients en améliorant leur compréhension et mémorisation. Il n'existe à ce jour pas de protocole d'élaboration de fiches conseils standardisées disponibles en soins primaires. Objectifs : - L'objectif principal est l'élaboration d'un protocole de création de fiches conseils standardisées à destination des patients rencontrés en soins primaires, se basant sur des données probantes. - L'objectif secondaire est d'établir la faisabilité du projet par le biais d'une étude de l'acceptabilité des auteurs cibles : les internes de médecine générale. Résultats : - Objectif principal : après le choix du thème à partir d'une banque de thèmes mise à jour régulièrement, les auteurs analyseront les documents existants, et réaliseront une synthèse de l'information circulante sous forme de revue de la littérature, sélectionnant un niveau de preuve minimal. Les exigences des patients seront recueillies via une étude qualitative (entretiens semi-dirigés). Les informations probantes seront sélectionnées, et les attentes des patients seront prises en compte pour rédiger la fiche conseil, tout en appliquant les conditions de format et de lisibilité. La lisibilité sera évaluée par le test de Flesch, et la compréhension des patients par méthode qualitative (focus groups). Le comité éditorial jugera de la validité scientifique par le respect du protocole. Un spécialiste graphique validera la mise en page, puis une traduction en trois langues sera réalisée. La fiche conseil sera diffusée sur un site internet dédié, et remise à jour tous les 3 ans. - Objectif secondaire : 39 internes ont répondu à un questionnaire diffusé en congrès, après présentation orale du protocole. Un document d'information écrite leur manque souvent (53,8%) voire très souvent (28,2%), la demande vient rarement (61,5%) des patients. Les fiches conseils sont considérées utiles avec certitude (66,7%). 41% des internes seraient intéressés pour participer au projet. Discussion : dans l'idée d'une approche centrée patients, leur avis est recueilli par méthode qualitative concernant le recueil de leurs attentes et leur compréhension. Le complément oral reste indispensable. L'impact des fiches conseils en conditions réelles sera nécessaire pour évaluer l'efficacité de ce protocole. Conclusion : la présente thèse a permis l'élaboration d'un protocole de création de fiches conseils pour les patients en soins primaires.
