La chirurgie thoracoscopique est responsable de 22 à 63% de DCPO. Nous avons émis l'hypothèse que l'ALR dans la chirurgie thoracoscopique pouvait avoir un impact positif sur la survenue de DCPO. Méthodes : Nous avons contacté et envoyé un questionnaire de douleur chronique (douleur, intensité, DN2, QDSA, NPSI) à 256 patients opérés d'une thoracoscopie de 2012 à 2014. Nous avons reçu 88 réponses exploitables. Les patients ont été répartis en 2 groupes : un groupe sans ALR (ALR- ; n = 36) et un groupe avec ALR (ALR + ; n = 52). Les dossiers médicaux étaient explorés pour relever le type d'ALR, la douleur aiguë, la consommation en morphiniques et en tramadol. L'objectif principal était de comparer la survenue des DCPO à plus de 3 mois dans le groupe ALR- versus ALR+. Les objectifs secondaires étaient de comparer l'intensité de la douleur précoce, la consommation en antalgiques opioïdes et les caractéristiques des DCPO. Résultats : Les deux groupes étaient comparables sauf pour la prémédication à la gabapentine, le nombre de trocarts et le délai entre la chirurgie et le questionnaire. L'incidence des DCPO était de 49% dans notre étude. Il n'y avait pas de différence entre les groupes ALR- et ALR+. Il n'y avait pas de différence de consommation de morphiniques per et post-opératoires, ni d'antalgiques de palier II, ni de douleur aiguë. Les scores d'évaluation des DCPO étaient similaires dans les 2 groupes. Conclusion : L'ALR n'a pas montré dans cette étude rétrospective d'intérêt dans la réduction des DCPO.
