Contexte : Le recours de plus en plus fréquent à des traitements anticancéreux oraux responsables de nombreux effets indésirables nous ont conduit à étudier la gestion de ces thérapeutiques par les patients ambulatoires. Objectifs : mesurer de manière qualitative l'intérêt d'une consultation pharmaceutique régulière dans le suivi ambulatoire des patients cancéreux traités par chimiothérapie et hormonothérapie par voie orale. Méthode : Deux guides d'entretien pharmaceutiques standardisés ont été formalisés à partir des données de la littérature. Ces référentiels ont permis d'interroger, selon un mode semi directif, une cohorte de patients cancéreux traités par voie orale. Le premier entretien se déroulait lors d'une rencontre avec le patient, puis le suivi était téléphonique. Résultats principaux: Le dispositif a été testé auprès de 28 patients sur une période de 6,5 mois, avec une moyenne de 3 entretiens réalisés par patient. 22 patients avaient recours à l'automédication; 18 aux médecines alternatives et complémentaires ; et 10 présentaient une observance incomplète. Les interventions pharmaceutiques ont concerné la prévention et la prise en charge des effets indésirables des anticancéreux chez 28 patients. Discussion-conclusion : Les anticancéreux par voie orale constituent une source de risques pour le patient, en termes d'interactions médicamenteuses, d'effets indésirables, de mode de prise inadapté et de mauvaise observance. Il est nécessaire de sécuriser la prise de ce type de médicaments via un dispositif d'éducation thérapeutique spécifique. Des études complémentaires doivent être menées, afin d'intégrer dans une démarche pluridisciplinaire, la prise en compte des représentations et des comportements des patients face à leurs traitements.
