Introduction : Les soins palliatifs à domicile interrogent toujours autant les médecins généralistes malgré de nombreuses évolutions législatives, visant à guider leurs pratiques. Quels sont aujourd'hui les besoins ressentis par les médecins généralistes isséens dans le vécu de leurs pratiques palliatives à domicile ? Matériel et méthodes : Une étude qualitative d'approche phénoménologique a été menée, sous forme d'entretiens individuels semi-dirigés auprès de médecins généralistes d'Issy-Les-Moulineaux, entre mai 2017 et janvier 2019. Résultats : 11 médecins généralistes isséens ont été interrogés : 7 femmes et 4 hommes, âgés de 29 à 67 ans, tous libéraux. Cette étude a permis de mettre en avant la relation médecin-malade en soins palliatifs, en décrivant la place du médecin généraliste, le type de relation et le cadre de soin. La prise en charge se faisait en plusieurs temps : discerner le « passage » en soins palliatifs, décider de la prise en charge, accepter et faire accepter l'absence de guérison et enfin se détacher de l'accompagnement. Des besoins essentiels autour de ces prises en charge ont pu être identifiés : un partenariat étroit avec une équipe experte en ville et des paramédicaux investis, une collaboration efficiente avec les services sociaux et hospitaliers, et le besoin de disposer d'un espace neutre d'échanges de pratiques. Conclusion : Cette étude propose des pistes d'amélioration pour les soins palliatifs à domicile, en ciblant spécifiquement les besoins exprimés par les médecins généralistes à travers leurs expériences de soins. Elle guidera également un projet associatif communal.
