Les directives anticipées permettent aux patients d'exprimer leurs volontés concernant leur fin de vie. Elles sont apparues pour la première fois en 2005 puis ont été modifiées en 2016 par la loi Claeys-Léonetti. Cette loi renforce l'autonomie du patient et leurs volontés s'imposent au médecin. Le médecin traitant se doit d'informer ses patients de l'existence de ces directives anticipées, ce qui lui donne un rôle clé. Les directives anticipées sont mieux connues des médecins généralistes et de la population française. Cependant encore trop peu d'entre eux les ont rédigées. En effet, il peut être difficile pour le médecin d'accompagner la fin de vie lorsque son but a toujours été de guérir. L'objectif de notre étude est d'évaluer les différentes méthodes utilisées par les médecins traitants pour promouvoir et accompagner à la rédaction des directives anticipées. Une étude qualitative par entretiens semi-dirigés a été menée auprès de médecins généralistes de la région Auvergne Rhône-Alpes recrutés pour une partie avec l'aide de l'URPS : Ce travail de thèse montre que les médecins traitants ont un rôle clé dans l'élaboration et la discussion des directives anticipées, c'est une figure de confiance. Il existe de nombreux freins concernant cette discussion sur la fin de vie aussi bien pour le patient que pour le médecin mais bien que difficile à aborder, échanger sur la fin de vie apporte un véritable apaisement à la fois pour le patient, ses proches que pour le médecin. Notre étude a montré qu'il existe de nombreuses perspectives d'amélioration aussi bien dans les outils utilisés qui sont nombreux que dans la façon de faire. De nombreux outils et façon de faire sont à notre disposition pour promouvoir les directives anticipées et permettre un meilleur accompagnement de la fin de vie. Le médecin traitant à un rôle central à jouer afin de permettre à son patient d'être acteur de sa fin de vie.
