La lésion périapicale, dénommée LIPOE (lésion inflammatoire périradiculaire d'origine endodontique), est une lésion inflammatoire associée à une infection intra-canalaire. En présence d'une lésion apicale, le traitement ou le retraitement endodontique dit traitement par voie orthograde, est la thérapeutique de choix de première intention. Mais ce dernier ne permet de gérer que les problèmes intra-canalaires. Les causes extra-canalaires et les causes intra-canalaires inaccessibles par la voie conventionnelle, ne peuvent être appréhendées que par la voie chirurgicale. On distingue aujourd'hui deux approches en chirurgie endodontique. La chirurgie apicale, en complément du traitement endodontique, permet de remettre la dent dans un contexte biologique favorable à la cicatrisation. Elle se limite aux trois derniers millimètres apicaux. Lorsque, pour diverses raisons, il est difficile de réintervenir par voie orthograde, la chirurgie est indiquée en première intention. On réalise alors un retraitement par voie rétrograde. Celui-ci consiste à désinfecter et à mettre en forme le canal sur toute sa hauteur, puis à obturer ce dernier, par voie chirurgicale. Parce que cette approche est techniquement possible, on parle dorénavant d'endodontie chirurgicale. Au-delà d'une seule considération sémantique, l'évolution est réelle.
