Ten years after the discovery of prostacyclin, our knowledge of its biochemical mode of action, pharmacological properties, pathophysiological significance and therapeutic applications is ever expanding. Prostacyclin is both complex and unique as demonstrated by its unusual feature of being chemically and meta bolically unstable when compared to other prostanoids and known amine or peptide mediators. Although physiologically essential, the chemical instability of prostacyclin poses a serious drawback in laboratory and clinical studies. It is one of the genuine objectives of pharmaceutical research to supply synthetic compounds which overcome the inherent drawbacks - considering investigational and therapeutic use - of endogenous compounds. Whereas metabolic instability in certain cases could be of advantage, chemical instability definitely is not. With Iloprost, a molecule has been designed which - according to all data so far available - pertains high receptor affinity, metabolic instability (clinically this equates with a fine control of Iloprost's effects) while chemical stability has been achieved. By virtue of these characteristics Iloprost can be considered as one step towards specific interaction within the arachidonic acid cascade, namely the prostacyclin receptor. The aim of the symposium was to provide a critical experimental appraisal concerning the biochemical mode of action and pharmacological properties of prostacyclin and Iloprost.

In June, 1981, we conducted a two day international symposium in Rome devoted to original scientific presentations on arachidonic acid metabolism in the kidney. Scientists from 20 countries were represented either in the general scientific program or in the poster presentation. This book does not represent the proceedings of this international gathering, but rather the edited manuscripts specifically prepared as sum maries of the scientific presentations. We feel that the timeliness and originality of the contributions as well as the need for a compilation of existing work on prosta glandins, thromboxane, and the kidney warrant this publication. The voluminous literature on prostaglandins includes some excellent reviews. For some time however, there has been a need for the collection in one volume of information from individual experts actively engaged in different aspects of prostaglandin research. This book is an attempt to fulfil that need. In view of the expanding interest and ever-growing accumulation of data of the various effects of prostaglandins it is hoped to revise the book and bring it up-to-date periodically. The largest section is that on reproduction, in which field progress in research and clinical applications thereof have been, and are, of great significance particularly in the area of popula tion control. This is not to decry the skill and efforts of investi gators in other aspects of prostaglandin activity nor the potential biological or therapeutic importance of their findings. In this context, what may appear to be omissions are individual sections on prostaglandin activity in the central nervous system, in the eye and as mediators of the inflammatory response. These are considered in the chapter on General Pharmacology of the Prostaglandins not because they might be considered to be of lesser impact and significance but in order to achieve a rapid publication of the book.
