Contexte : Suite à l'étude HAPO qui montre un lien direct entre hyperglycémie et morbidité périnatale, l'IADPSG a proposé sur consensus de nouveaux critères diagnostiques. Reprenant ces critères, les recommandations françaises de 2010 préconisent un dépistage ciblé par une glycémie à jeun (GAJ) précoce et suivie, si normale, par une HGPO à 75g de glucose à 24-28 semaines d'aménorrhée (SA). L'objectif de cette étude est de déterminer les caractéristiques maternelles et les issues de grossesse associées à un diagnostic précoce du diabète gestationnel. Méthode : Etude menée chez des patientes dépistées sur facteurs de risque dont le suivi diabétologique et obstétrical est réalisé au CHRU de Lille de février 2011 à juillet 2013. Le diagnostic de diabète gestationnel est posé si la GAJ précoce est ≥ 0.92g/l ou si au moins une valeur est de l'HGPO est anormale (≥ 0.92g/l, ≥1.80g/l à 1 h et ≥1.53g/l à 2 h). Quatre groupes sont déterminés en fonction du terme du diagnostic pour l'analyse comparative (15 SA, 15-23 SA, 24-28 SA et 28 SA). Résultats : 1077 patientes sont incluses. 28.3% sont diagnostiquées avant 15 SA, 10.7% entre 15-23 SA, 45.4% entre 24-28 SA et 15.5% après 28 SA. Les patientes diagnostiquées précocement ont un indice de masse corporelle plus élevé (p0.0001) et ont plus souvent recours à l'insuline (p0.0001). Les taux de macrosomie sont de 14.8% ( 4000g) et de 18.1% (selon Gardosi). Entre 2011 et 2013, le taux de macrosomie se réduit à 10.4% pour le groupe 24-28 SA, significativement pour la macrosomie selon Gardosi (p=0.0336).
