L'apport du laser pour améliorer la prise en charge du patient et la qualité de l'acte endodontique représente un enjeu. Afin d'utiliser correctement les lasers et obtenir des résultats cliniques satisfaisants, il est indispensable de comprendre les principes physiques. Au travers de cette revue de la littérature, nous étudierons si le laser, en complément des désinfections chimio mécaniques conventionnelles, a un réel intérêt afin d'améliorer la désinfection canalaire et l'élimination de la boue dentinaire sur des dents humaines avec des conditions péri apicales normales ou infectées. Ainsi, l'utilisation du laser en endodontie peut être considérée comme un adjuvant éventuel à la désinfection canalaire et à l'élimination de la boue dentinaire et non comme une alternative à l'irrigation conventionnelle, le niveau de preuve des études restant actuellement faible.

Over the past ten years, carbon dioxide laser surgery has made impressive strides and is now applied to every field of surgery without exception. It is the intention of this book to record the work done in this field in the Department of Plastic and Maxillofacial Surgery of the Beilinson Medical Center and Tel Aviv University Medical School, Israel, as well as that performed in association with other depart ments. In this context, one feels that it is incumbent upon one to acknowledge the cooperation of the medical and paramedical staff of the Department of Plastic and Maxillofacial Surgery of the Beilinson Medical Center, as well as that of Prof. Yehuda Shindel and Dr. Daniel Katenelson of the Department of Ear, Nose and Throat, Dr. Y ona Tadir of the Department of Obstetrics and Gynecology and Dr. Itamar Kott of the Department of General Surgery. The progress of Laser Surgery is well demonstrated by the participation in the four meetings of the International Society for Laser Surgery, the first of which was held in Tel Aviv in 1975 with an attendance of 65 and the last in Tokyo in 1981 with an attendance of 1200.

La colonisation bactérienne est la première cause du développement des maladies pulpaires et péri-apicales. Le traitement endodontique doit allier une mise en forme canalaire et une irrigation optimale pour améliorer l'élimination du biofilm bactérien installé. L'hypochlorite de sodium est aujourd'hui l'irrigant de choix en endodontie. Plusieurs méthodes permettent de l'activer pour potentialiser ses effets, dont l'activation photonique. Le laser Er : YAG permettrait de rendre plus actif l'hypochlorite de sodium, et de favoriser la désorganisation du biofilm bactérien par propulsion de la solution. Nous avons réalisé une étude expérimentale ex vivo afin de mettre en évidence cette action présumée du laser Er : YAG.
