La convention pharmaceutique de 2012 a initié des changements dans la rémunération du pharmacien d'officine tout comme dans la pratique de son exercice. Ces changements étaient attendus par une grande partie de la profession qui voyait sa rémunération diminuer. Il fallait donc évoluer et révolutionner la rémunération pour que le réseau officinal soit pérennisé. Malgré tout, cette Rémunération sur Objectifs de Santé Publique ne permet pas d'obtenir une rémunération pérenne et cohérente, selon la plupart des représentants de la profession pharmaceutique. En effet son niveau devrait diminuer, jusqu'à devenir très faible en raison de l'augmentation du taux de substitution par les spécialités génériques. L'honoraire à la boite déplace seulement le problème lié à la perte de marge commerciale sur le volume de boites de médicaments. Il valorise peu l'acte intellectuel. Les mesures de santé publique envers les médecins visent pourtant à diminuer les volumes de boites dispensées. En parallèle le pharmacien d'officine cherche à dispenser le plus petit nombre nécessaire de médicament pour les patients. L'honoraire pour les ordonnances complexes, valorise l'acte intellectuelle mais reste peu significatif. En définitive, cette évolution de la rémunération peut paraître décevante. Le mode de rémunération devra donc évoluer ces prochaines années pour une meilleure reconnaissance du conseil pharmaceutique.

Le métier de pharmacien d'officine est en pleine mutation dans un environnement de déficit chronique de la Sécurité Sociale et d'érosion de la rentabilité financière des pharmacies. La Convention Pharmaceutique de 2012 et l'avenant n°11 marquent un grand virage qui modifie radicalement le mode de rémunération des pharmaciens et qui propose un panel de services rémunérés. Cette mutation en marche en France est à l'image de la multiplication des services développés à l'étranger (Royaume-Uni, Allemagne, Québec et Suisse). C'est dans ce contexte que nous avons voulu mener une étude qualitative semi-directive pour comprendre dans quelles mesures les pharmaciens suivaient les propositions de nouveaux services et comment le nouveau mode de rémunération était-il perçu après dix-huit mois de mise en place de l'avenant n°11. Nous avons réalisé une analyse triangulée des verbatims en menant une analyse individuelle puis une discussion autour des convergences et des divergences. Ceci nous a permis de constater bien des écarts entre les attentes respectives de la Sécurité Sociale et des pharmaciens. Les résultats reflètent une grande inquiétude chez les pharmaciens interrogés en raison d'une rémunération jugée limitée, d'une charge administrative trop contraignante, d'un besoin d'effectifs supplémentaires et d'un réaménagement de l'espace nécessaire. De plus, ils craignent l'émergence de conflits avec les autres professionnels de santé, la rupture du monopole et la disparition des petites officines. La majorité des pharmaciens interrogés pourtant volontaires à la vaccination, n'ont pas adhéré aux autres missions. Cependant, les pharmaciens sont motivés et désireux de saisir ces opportunités de nouveaux services à condition d'un réaménagement du travail administratif et de la rémunération. La satisfaction et la fidélisation des patients sont au centre des préoccupations des pharmaciens. Les nouvelles missions sont reconnues comme étant un moyen de parfaire leur image de professionnel de santé et de diversifier leurs activités et leurs sources de financement. Les chiffres publiés pour les deux premiers mois de 2019 par l'Union des Syndicats de Pharmaciens d'Officine, montrent des résultats encourageants avec une nette amélioration de la rémunération des pharmaciens. Cependant, les pharmaciens attendent la fin 2020 pour apprécier définitivement les dispositions de l'avenant n°11.

Countries that have made the most progress toward universal coverage have public expenditures in health equivalent to at least 6% of their gross domestic product (GDP), which is the percentage established in PAHO's universal health strategy as the benchmark for the countries. However, while higher expenditure is a prerequisite, it is not enough to combat inequities and advance toward universal health. In addition to greater resources, the quality of the expenditure must be improved, reducing health system inefficiencies. Moreover, public expenditure in health should be sustainably increased in a fiscally responsible manner. The concept of fiscal space for health refers to the ability of governments to provide additional budgetary resources for the health system without affecting the financial position of the public sector or supplanting other socially necessary expenditures. Any analysis of fiscal space, therefore, will attempt to identify the prospects for increasing health expenditure in the short and medium term to address a series of clearly established health needs. These efforts are under way at a critical time in the Region of the Americas, particularly in the countries of Latin America and the Caribbean, which are engaged in a singular health system reform process. For the first time in history, these countries have formalized their intention of increasing public expenditure in health, putting themselves firmly on the path to real and effective access to health care through the universal health strategy. Without achieving basic well-being at this level, it will be impossible to improve social cohesion and social development in the countries of the Region.
